UHC activities and YaKaP Registration CY 2025
Date : DECEMBER 12, 2025
Time : 8am to 3pm
Venue : The Grand Beanery ( Along National Highway, in front of Old Municipal Hall)
List of Requirements for New Member/Registration
Senior Citizens
- PMRF (properly filled up and signed by the member)
- SENIOR ID or any valid ID with date of birth (photocopy)
- BIRTH CERT/MARRIAGE CONTRACT with Registry number (photocopy)
PWD
- PMRF (properly filled up and signed by the member)
- PWD ID (photocopy of front and back)
- Must be included in the PRPWD (system of DOH- please confirmed it to PDAO / DSWD of your LGU)
4Ps
- PMRF (properly filled up and signed by the member)
- 4Ps ID (photocopy)
- Certification of active member (DSWD Municipal link)
Solo Parent
- PMRF (properly filled up and signed by the member)
- Valid Solo parent ID (photocopy of front and back)
First time Job seeker (bagong graduate na wala pang PhilHealth Number)
- PMRF (properly filled up and signed by the member)
- Valid ID or Birth Certificate (photocopy)
- Cert of First time Jobseeker (signed by the Barangay Captain - we need photocopy only but pls bring the original copy)
Para sa magpapatransfer (from other YAKAP to Addere YAKAP) - Certificate of Transfer
Para sa magrerequest ng MDR- pakiphotocopy ng any valid ID.
Para sa magrerequest ng ID- pakiphotocopy ng any valid ID at gumawa ng request letter kung para saan ang nirerequest na ID.
Kung magdadagdag ng dependent:
Anak: must be 0-20 years old only
- PMRF (properly filled up and signed by the member)
- Any Valid ID (photocopy)
- Birth Certificate with registry number (photocopy)
Asawa: must be legal
- PMRF (properly filled up and signed by the member)
- Any Valid ID (photocopy)
- Marriage contract with registry number (photocopy)
