When an Orthosis Becomes Part of the Problem
This patient presented with a significant knee hyperextension deformity (genu recurvatum) that had developed while using a poorly designed orthosis.
An orthosis is not simply a brace that holds a limb in place. Its design, alignment, trimlines, joint positioning, and interaction with the patient's biomechanics can profoundly influence gait and joint loading.
In this case, an inappropriate orthotic design contributed to abnormal forces around the knee, progressively encouraging hyperextension rather than controlling it.
The lesson is simple:
A brace must be designed for the patient—not just for the diagnosis.
Every orthosis should be carefully assessed for: • Alignment
• Joint control
• Ground reaction forces
• Muscle strength
• Gait pattern
• Functional requirements
A poorly designed orthosis can create new problems. A well-designed orthosis can restore function, protect joints, and improve quality of life.
Precision in orthotic design is not an option. It is the treatment.

Charcot Foot: When Diabetes Quietly Destroys the Ankle

Charcot foot is one of the most serious complications of diabetic neuropathy. Because the protective sensation is lost, repeated walking on an injured foot can cause the bones and joints to weaken, fracture, and collapse without significant pain.

When the ankle joint is affected, the consequences can be severe:
• Progressive instability of the ankle
• Joint dislocation and deformity
• Collapse of the foot and ankle alignment
• High risk of pressure ulcers and infection
• Increased likelihood of amputation if left untreated
