Neer Test
The Neer test is a clinical test used to assess subacromial impingement of the shoulder, particularly irritation involving the supraspinatus tendon and/or subacromial bursa.
Procedure:
1. Patient sits or stands.
2. Stabilize the scapula with one hand.
3. Internally rotate the patient’s arm (thumb pointing down).
4. Passively elevate the arm in forward flexion to the end range.
Positive test:
Pain reproduced, typically between about 70°–120° of elevation, suggests subacromial impingement.
Structures commonly implicated:
* Supraspinatus tendon
* Subacromial bursa
* Long head of biceps tendon
Clinical point: A positive Neer test is not specific for one particular pathology, so it should be interpreted along with other shoulder tests and the patient’s history.
