Alhmdulilah an interesting and difficult case done by Urology dept,Hameed Latif Teaching Hospital, Lahore.

An old age pt presented with hx of Rt flank pain from last 4 to 5 years, hx of frequent pain killer intake- +ve

On Ct scan,there was Rt 180° malrotated kidney with 2 stones at PUJ

On HRCT,there was Lt partial lung collapse due to some old pulmonary TB,so high risk for G/A from anesthesia POV.

Rt Prone PCNL under spinal anesthesia planned for the safety of Pt.

Alhmdulilah complete clearance achieved and pt is stable from every aspect.

But there were also multiple secondary stones in lower + upper pole..
