When you sit for long periods, your body weight is continuously applied to the gluteal region. This creates sustained pressure on the soft tissues underneath, including branches of the sciatic nerve.

At the same time, the gluteal muscles become less active because they are not required for stabilization in a seated position. This combination of reduced muscle activation and continuous pressure can lead to a numb or “dead” sensation in the glute area.

This is not a structural injury in most cases — it is a functional response to prolonged static loading and reduced movement.

When you sit in a slouched position for extended periods, the pressure on the lower lumbar spine increases dramatically. The L4–L5 disc is one of the most vulnerable areas because it carries a large portion of body weight and movement stress.

Over time, this constant pressure begins to weaken the outer layer of the disc. The inner material slowly pushes outward, creating a bulge or herniation. This herniated disc can then press on nearby nerve roots, leading to pain that radiates from the lower back into the leg.

🛑 Stop aggressively rolling a lacrosse ball into your glute if you have sciatica.

That sharp, burning pain shooting down your leg isn’t always a spinal disc issue—it may be Piriformis Syndrome (Deep Gluteal Syndrome), where a tight hip muscle compresses the sciatic nerve.

What’s happening?

The sciatic nerve runs beneath (or through) the piriformis muscle.

Prolonged sitting weakens the glutes.

The piriformis overcompensates → goes into spasm.

This creates a “chokehold” on the nerve → causing radiating leg pain.

Why lacrosse ball grinding makes it worse:

You’re pressing an already compressed nerve against a hard surface—increasing irritation and pain, not relieving it.

What to do instead:

1. Release the front (not the pain site)
Foam roll hip flexors and TFL (front/side hip) to reduce tension pulling on the piriformis.

2. Gentle stretch (Figure-4)
Cross ankle over knee, lean forward with a straight spine. Hold 30 seconds.

3. Strengthen glutes (Clamshells)
Activate glute medius to reduce overload on piriformis.

WHY YOUR BACK PAIN WHEN STANDING IS NOT A CORE PROBLEM ⚡️

Lower back pain that appears when standing is often blamed on a weak core.

However, research from the Mayo Clinic, APTA, and JOSPT shows that one of the most common causes is limited hip mobility, not lack of strength.

When the hips cannot extend properly, the pelvis shifts into an anterior tilt position.

This changes the alignment of the spine and increases the arch in the lower back.

As a result, more load is transferred directly to the lumbar spine instead of being shared through the hips.

Over time, this creates compression, fatigue, and pain in the lower back during standing.

What makes this misleading is that the pain is felt in the back, leading many people to focus only on core strengthening.

But if the hips are restricted, the spine is forced to compensate.

Have you ever noticed **your shoulder blade sticking out when pushing against a wall**?

For some people, this unusual movement may appear during activities such as:

• pushing objects
• lifting the arm forward
• doing push-ups
• raising the arm overhead

Sometimes the shoulder blade may appear **unstable or winged**.

🦴 THE ANATOMY

One nerve responsible for stabilizing the shoulder blade is the **Long Thoracic Nerve**.

This nerve travels along the side of the chest and supplies the **Serratus Anterior** muscle.

The serratus anterior connects the ribs to the **Scapula** (shoulder blade).

Its main role is to keep the scapula firmly positioned against the rib cage.

⚙️ THE BIOMECHANICS

During movements such as pushing or raising the arm, the serratus anterior helps rotate and stabilize the scapula.

This stabilization allows the shoulder joint to move efficiently.

If the long thoracic nerve becomes irritated or injured, the serratus anterior may weaken.

As a result, the shoulder blade may move abnormally and appear to stick outward.

⚠️ WHEN TO PAY ATTENTION

Changes in shoulder blade movement may indicate muscle weakness or nerve irritation.

Symptoms may include:

• shoulder blade protruding outward
• difficulty lifting the arm overhead
• weakness during pushing movements
• shoulder fatigue during activity
