Flexion Rotation Test (FRT)
Why it matters clinically 🧠

The Flexion Rotation Test is a highly valuable tool to assess upper cervical mobility, specifically targeting the C1–C2 segment, which contributes to nearly 50% of total cervical rotation.

In full cervical flexion, lower cervical segments are mechanically restricted, allowing the clinician to isolate movement at C1–C2. This makes the FRT particularly useful in identifying dysfunction that may not be evident in general ROM testing.

✅ Clinical significance: • Strongly associated with cervicogenic headache (CGH)

• Helps differentiate CGH from migraine and tension-type headaches
• Identifies upper cervical hypomobility/asymmetry
• Guides targeted manual therapy and exercise prescription

Cervical Distraction Test

More than just a maneuver, this test helps you understand why a patient’s symptoms change. By gently unloading the cervical spine, you’re essentially testing whether symptoms are driven by neural compression.

If a patient’s arm pain or paresthesia eases during traction, it points toward a compressive pathology rather than purely muscular pain.

🔎 Clinical reasoning matters:
Pair it with Spurling’s test — one provokes, the other relieves. Together, they give you a much clearer picture of cervical radiculopathy.

⚠️ Always screen first. In cases of instability, trauma, or compromised bone quality, this test should be avoided.

Build your assessments around interpretation, not just technique—that’s what separates a student from a clinician.
