Have you ever grieved a patient who didn't die? Dr. Hailey Evans interviewed 41 neonatologists across Massachusetts and found something clinicians rarely say out loud: loss in the NICU isn't only about mortality. A baby who fails extubation, develops NEC, or survives with a serious morbidity can trigger the same grief response as a death. Twins where one survives. Families who fought through IVF only to face a hard outcome. These cases stay with people the same way a death does, even when everyone else moves on because "the baby lived."

Sometimes the baby survives and the loss still lands.

Most debriefs and M&Ms cover physiology and timeline, rarely the part where you go home replaying what you felt. Evans found that unsupportive culture, not the loss itself, is often what keeps clinicians from processing it. Worth sitting with before your next case review.
