A Class II restoration can be technically correct… and still anatomically wrong. 🦷

So, what really makes the difference?

Look closely at the sectional matrix.

Its curvature and convexity are not random details — they are the blueprint for the proximal anatomy we are trying to reproduce.

A well-adapted matrix helps us create:

→ A natural proximal convexity
→ A controlled contact area
→ Proper embrasure form
→ A smooth emergence profile
→ A cervical contour that is functional, cleansable and biologically respectful

But the matrix alone doesn't do the job.

The restoration is built layer by layer — first establishing the proximal wall, then progressively reconstructing the missing anatomy, followed by careful finishing and polishing to refine the transition between the composite and the natural tooth.

The objective is not simply to close the proximal box.

It is to make the restoration behave like a natural tooth.

Because a good Class II composite isn't about how much composite you place…

It’s about how accurately you rebuild what was lost.

Proximal anatomy is not an accessory.
It is part of the restoration.

Class II Composite | Rebuilding what was lost — anatomically. 🦷

A predictable posterior composite starts with one important principle:

Build the proximal wall before you build the tooth.

The missing proximal structure was reconstructed incrementally, carefully controlling the emergence profile, contact area, embrasure and relationship with the adjacent tooth.

The goal?

Not simply a “closed” contact — but a natural, functional and cleansable proximal anatomy.

Small increments. Precise walls. Controlled contact. Natural anatomy.

That’s where direct composite becomes restorative dentistry — not just filling a cavity.

This case was restored in 2018 using direct composite veneers with Tokuyama composite, replacing old composite restorations in a single appointment.
The patient returned in 2026 — 9 years later.
No replacement.
No major intervention.
And, unfortunately, no maintenance visit for more than 8 years.
Yet the restorations remained clinically functional with satisfactory esthetic stability.
This case is not about claiming that every composite veneer will last 9 years.
It's about something more important:
When biology, adhesion, anatomy, finishing, occlusion and patient factors are respected, direct composite can go much further than we sometimes expect.
2018 → 2026
9 years. Still there. Still functioning.
