Peria**l swelling/discharge isn’t always “piles.”
Many people feel shy to talk about symptoms around the a**s, so they delay care. But if you have recurrent swelling, pain that comes and goes, or persistent discharge, it may not be hemorrhoids. One common cause is an a**l fistula — a small abnormal tunnel that often develops after a peria**l abscess.
Symptoms that should raise suspicion:
Recurrent swelling near the a**s (may burst and then improve)
Persistent/intermittent discharge (pus/serous), soiling, bad smell
Pain that worsens during swelling (sometimes an abscess is forming)
History of previous abscess drainage
Seek urgent medical care if you have: 🚩 Fever with severe pain
🚩 Rapidly increasing swelling
🚩 Severe pain in a diabetic/immunocompromised patient
🚩 Non-healing wound or unexplained weight loss/altered bowel habits
Why early assessment matters:
Early diagnosis and correct treatment can prevent repeated infections and long-term discomfort.
Clinical note (for doctor colleagues):
In suspected complex/recurrent cases, MRI pelvis with fistula protocol is very helpful for mapping the tract, internal opening, and secondary extensions before definitive management.
