CREATION OF MERCI SIWAINAO CLINIC'S MEDICAL LABORATORY

A story of crafting the services to patients' needs. We are still evolving.

Got a call on 19/03/2024.

"Hello, Dr Jack. I am already retired. I wanna come and help you, at your clinic." Said a retired Lab technician.

"Come and see me on Saturday." I replied.

We met on Saturday 23/03/2024, at Merci Siwainao Clinic.

"Come and start next Saturday. I will book cases for you. For a start, you can use the birthing room, on Saturdays. We will build a Lab for you, in the next few months." I said.

We commenced doing semen analysis on 30/03/2024, as part of our fertility investigations. We started off, with just a microscope. The only medical equipment, we have.

Our clinic is usually busy on Saturdays as General outpatient, Gynae, Antenatal, VIA clinics, and checks for men, run simultaneously.

I have observed that our men lack privacy, in having to use the clinic's rest room to produce the required specimen. Thus, taking longer, with a few unsuccessful attempts.

"Uncle John (my builder) bae umi need for wakem wanfala room for oketa man, inside lo Medical Lab ya, for privacy and oketa mami too, save go in for help oketa husband blo oketa too. (We need to make a room for the men in the Medical Lab, for privacy, and to enable the wives to go in, as well to assist their husbands in producing the needed specimen." I explained to my builder.

Construction work commenced in, late May 2024.

We started using our Lab today.

"Dr Jack, me sapris, today, fast tumas man. Oketa man ya go in, no long time nomoa oketa come out too. (The men were very quick, today)." The technician exclaimed.

I would like to thank Uncle Sammy Waraikau Kaohi, who came all the way from the village (Hauhui Village, West Are Are, Malaita Province) to lay the foundation of the Lab.

I would like to thank, Mike especially for the frame and the finishing work, as well as Danny.

Thanks to Oki, Naitoro, Sam, Clarence Penny and Franet Iakolo for helping out as well.

Foremost, I would like to thank Uncle John Masol the mastermind (the supervisor) - my designer, builder, electrician, plumber, welder, procurement officer, driver etc ....my everything hahaha. You are indeed the miracle maker, who turned my words and ideas into reality.

I would also like to thank all the patients who have come and used our services. With your money paid for our services, we are able to expand the clinic, to help many more.

God Almighty, I thank You for giving me this vision, to serve the women (and their unborn babies) of this country. I believe that the women of this country, are no second class citizens. They are first class citizens. And they deserve FIRST CLASS SERVICE.

God, I thank You for the strength, wisdom, knowledge and guidance. God, this is all we have, and this is the best we can do. The rest is Yours. It is Your business to provide the machines we need to serve your people. You bring the Haematology, Biochemistry, TFT, PSA machines, the things to do transfusion services, and everything we need to serve our women and their babies.

A 28yrs old woman and her husband saw me in May, for fertility consultation. They've been together since 2013. I saw her (and her husband) again on 1/07/24. Her pregnancy test was positive. Ultrasound scan confirmed that she was pregnant. She began her booking antenatal visit with us.

Case 2 (pic 2)
A 27yrs old lady with her husband saw me on 11/03/24, seeking fertility assistance. She had surgery for an ectopic pregnancy (pregnancy formed in her right fallopian tube) in 2021. After that, her period became very irregular. Having prolonged periods. Coming on, twice a month.

I devised a management plan for them.

She (and her husband) came with a positive pregnancy test today. Ultrasound scan confirmed an intrauterine pregnancy. She is 8 weeks today - their best Independence present.

This is indeed a precious pregnancy. She began her antenatal visit with us, today.

President J.F Kennedy, in his inaugural address, on 20th January 1961, said, and I quote, "Ask not what your country can do for you - - ask what you can do for your country," end of quote.

Let ask ourselves, how we can individually serve our country.

As our motto says, "To lead is to serve."

Let us use all our gifts, talents, knowledge and whatever resources we have, to serve our people in anyway possible, we can, whether big or small.

If you are gifted in singing, use it to serve. If you are a builder, use your skills. If you can cook, find ways to serve. If you can motivate others, do that. If you have the gift to teach, use it.

Services to many, leads to greatness.

Our overseas partners will come to help us. But it will take us, Solomon Islanders, to build our country. Cos this is our country.

For 15yrs, serving in our public hospitals, I have complained and complained.

Now coming out to do private practice, I've come to appreciate what our government has done for us, giving us, free health care.

Medical drugs and medical equipment are very expensive.

We need a robust economy to increase government's the support for health, inorder to improve our services.

If you go to many countries in the world, you will realise that health care is not free. Very expensive, of course. And if you don't have health insurance, you won't access health care.

Been the Consultant In-charge of the National Referral Hospital's Labour Ward, I have seen mother and their babies sharing one bed (2 or 3 per bed).

I have seen women sitting in queues, for 2 - 5 hours to see me, at my Tuesday Specialist Gynae & Infertility Clinics.

Women (with high risk pregnancies), lining up on Thursdays for my ultrasound scan sessions, doing surveillance on their babies.

Referrals, allover the country coming to see you, in very limited time slots. You have to keep your consultation time to the least minimum time possible, cos they are just too many.

Difficult to do your best for them all, as this drains you, working their lunch. This is the type of work my colleagues continue to do at our public hospitals, serving our people.

To make it worse, all of us, specialists have signed contracts with the government, in which we are not allowed to do private practice after hours, or even when on leave. Making our specialist services, inaccessible to the public.

Deciding to go into full time private practice, makes all my specialist Obstetrics and Gynaecological services, available to the public.

Merci Siwainao Clinic is only 14 months old.

A good number of our fertility patients are already pregnant. And a few have delivered, already.

Since January this year, we are delivered 24 babies, already. All our babies are well. And all our mothers have not developed infections (or any other complications) even up to 28 days after delivery, at the last postnatal check.

This how, I have decided to use my gifts, talents, knowledge and experience, to serve our people, in the private health sector. Complimenting the work of our government. Relieving some of the workloads at NRH, shouldered my by dear colleagues at the Labour Ward, and O& G Department, as a whole.

This is Public Private Partnership in health, for the betterment of our people.

You can access our services, Monday to Sunday, 9am to 9pm.

You can make an appointment or you can just walk in.

We do Malaria tests, General Outpatient Consultation, Specialist Gynae, Fertility check, Antenatal check, pregnancy scan, female pelvic scan, transvaginal scan every day.

On Saturdays, we do cervical cancer screening (VIA test) and treatment for women, and Semen Analysis test (s***m count), for men.

We are located at Henderson, down 9-1 road, opposite Henderson Plaza and Zome. (See arrows indicating the direction along the road)

2 PRECIOUS BABIES BORN AT MERCI SIWAINAO BIRTHING CENTER.

1. A 36yrsold, booked at Merci, at 16wks (4 months) with some bleeding. She has had 2 previous miscarriages, and subfertility for 4yrs.

At 35weeks, her baby was breech (buttock-first). She was really scared of having a caesarean section. I successfully, turned (External cephalic version - ECV) her baby, around, to be head-first. She was very happy.

On 25th May 2024, at her 12th visit, at 40wks5days, she had experienced reduced fetal movement (risk of her baby dying).

CTG test was reassuring and the Umbilical Artery Doppler was normal.

A Foley catheter was inserted for Induction of Labour.

Labour was augmented yesterday.
She delivered Princess Cedella, at 4:22pm, in excellent condition. She weighed 2,825grams.

They stayed overnight and were discharged, today.

2. A couple presented at my clinic, on 3/06/23, seeking help. They were married for 2yrs, and struggling to conceive.

She had been bleeding, for over a month.

We had to regulate her cycle (make it normal), and began fertility management.

They returned, on 22/08/23. They were given further instructions.

On 25/09/23, her pregnancy test was positive.

On 21/11/23, at 13weeks gestation, she presented to Merci, with a Threatened Miscarriage. We managed to pull her pregnancy through.

She had a total of 11 Antenatal visits.

Yesterday, she came in Active Labour, 4cm, at 2pm. CTG and Doppler UA were normal.

Labour was augmented.

Princess Victoria was delivered via assisted vacuum (pulled the baby out) due to prolonged 2nd stage and maternal exhaustion.

She came out in excellent condition, weighing, 3,875grams.

She and her mum were discharged this afternoon.

Thanks to the very supportive husbands and relatives who had helped us coach the mothers during birthing.

Thanks to my very hard working midwives.

All glory be to God for great things He has done.
