Why do and should pharmacies ask for prescriptions when patients want to buy Budecort (budesonide) products over the counter for use in nebulizers?

Budecort contains a drug called budesonide which is in a class of medicines called corticosteroids. It comes in several forms but in Seychelles, the most common form is a nebulization solution (0.5mg or 1mg) and is used to help PREVENT AND REDUCE the symptoms of asthma. It is NOT for the TREATMENT of asthma attacks. Corticosteroids work by reducing inflammation in the lungs that can cause an asthma attack. If used regularly and as prescribed by a doctor, they reduce the frequency and severity of asthma flare ups. They are NOT effective if an asthma attack has already started and the patient is wheezing.

There is a very big misconception in Seychelles. Many patients tend to buy Budecort over the counter in pharmacies for a simple flu or cough, especially in children, without the advice of a doctor. This is not what Budecort should be used for.

Overuse and misuse of such corticosteroids come with a risk. There is a risk that such medicines can slow growth, particularly in children, and in extreme cases, cause adrenal gland insufficiency. Children who are being treated with Budecort or any form of corticosteroids should be done so under the supervision of a doctor who will monitor their growth and other developmental factors. Not only this, but the doctor will also check to see that the medicine is doing what it is supposed to do! The medicine needs to be used at the lowest possible dose for the shortest time possible to control the symptoms of asthma.

It is important to note that corticosteroids such as Budecort nebulizing solution increase the risk of other infections. Let’s take a simple example: a child has a small cough and the parents are unaware that this cough is associated with a chest infection. They just think it’s a cough and there is nothing more to it. They purchase Budecort and use it when they think the cough is ‘bad’. After a few days they notice that their child’s cough is getting worse and a fever is developing. The reason for this is because the Budecort supresses the body’s natural defence system. Your body cannot fight off an infection thus allowing the infection to become worse. In extreme circumstances this can cause pneumonia to develop. Albeit the chances of this happening with inhaled corticosteroids are low, it is not something that should be overlooked, especially in patients where such drugs are being used ‘as and when’ based on their self-diagnosis and without supervision of a doctor. Asthma is something that needs to be managed over a period of time. It is not a condition that can be treated as a 'one off'.
