Information on the burden of postacute sequelae following SARS-CoV-2 infection (or long COVID) amongst immunocompromised cancer patients is limited during endemicity.

Researchers from National Cancer Centre Singapore, Singapore General Hospital and Duke-NUS Medical School collaborated to evaluate the risk of long COVID in a population-based cohort of Singaporean cancer patients during Omicron predominance, as part of a multi-centre project of the national Programme for Research in Epidemic Preparedness and REsponse (PREPARE), under the Communicable Diseases Agency.

Their findings suggested that amongst COVID-19 vaccinated/boosted cancer patients, overall risk of long COVID was not significantly elevated, highlighting the continued importance of COVID-19 vaccination amongst immunocompromised cancer patients during endemicity.

The anion gap (AG) is commonly used to screen for acid-base disorders. The diagnostic cut-off for high anion gap metabolic acidosis (HAGMA) was determined decades ago with earlier laboratory techniques. With modern laboratory methods, it was thought that the cut-off may have changed as measured levels of serum chloride may be higher. On the other hand, modern laboratory equipment are still calibrated to familiar reference ranges established with earlier techniques.

Researchers from Changi General Hospital (CGH) and SingHealth assessed the performance of AG as a screening test for HAGMA and to determine the optimal diagnostic threshold of AG for HAGMA.

The team analysed data from 16,475 patients and concluded that an AG threshold value of ≥15 mEq/L has a high sensitivity for HAGMA.

Congenital hyperinsulinism (CHI) is the commonest cause of persistent hypoglycaemia in neonates and infants. Diazoxide remains first-line therapy, but variability in response and adverse effects poses clinical challenges.

Researchers from KK Women's and Children's Hospital and Duke-NUS Medical School conducted a structured review on diazoxide pharmacokinetics, pharmacodynamics, and safety in neonatal and infant populations. They synthesised available evidence alongside developmental pharmacology principles to explore age-dependent variability in drug response and toxicity.

The review highlights the potential role of individualised dosing strategies based on clinical phenotype, genotype, and patient-specific risk factors, while acknowledging current evidence limitations. These findings support a more tailored approach to diazoxide use to optimise safety and effectiveness in CHI management.

Can an AI-powered eye scan help predict diabetic foot disease? 👁️🦶

Diabetic foot disease is one of the most common and serious complications of diabetes, often leading to foot ulcers, infections, and lower-limb amputations. Yet, identifying patients at risk early remains a challenge.

Researchers from Singapore National Eye Centre / Singapore Eye Research Institute, Singapore General Hospital and Agency for Science, Technology and Research (A*STAR), led by Assoc Prof Yu-Chi Liu, have developed an AI-powered approach that combines non-invasive corneal imaging with routine blood tests to predict an individual’s risk of developing diabetic foot disease.

Their shows that tiny changes in the nerves of the eye could provide an early window into diabetic nerve damage. By combining non-invasive eye scans with routine blood test results, the AI model identified patients at risk of diabetic foot disease with up to 94% accuracy, paving the way for earlier intervention before serious complications develop.

Ureteral stenting is widely adopted in kidney transplantation (KTx) to mitigate major urological complications (MUC); however, heterogeneous practices continue to shape clinical decision-making regarding its influence on infectious outcomes, optimal dwell time, preferred removal technique, and the merits of routine versus selective stenting strategies.

Researchers from Singapore General Hospital synthesised contemporary evidence to clarify best practices and identify future directions for stent use in KTx recipients.

Their key findings show that removing internal double-J stents 14–21 days after transplant using minimally invasive, office-based approaches strikes the best balance between preventing major urological complications and reducing UTI risk, and emerging tools like stent-on-string, single-use cystoscopy, and magnetic stents are reshaping patient experience and cost-effectiveness. The debate between routine and selective stenting remains ongoing, with personalised approaches on the horizon.
