Why You Shouldn't Pop Pimples

Popping a pimple may seem like the quickest solution, but it often creates more problems than it solves.

When you squeeze a pimple, bacteria, oil, and debris can be pushed deeper into the skin, increasing inflammation and making the lesion larger, more painful, and slower to heal. Breaking the skin barrier also increases the risk of infection.

Popping pimples can also lead to:

More inflammation, which may worsen the breakout.
Spread of bacteria to nearby pores, triggering new pimples.
Post-inflammatory hyperpigmentation (PIH), causing dark marks that can persist for months.
Acne scarring, which is often much more difficult to treat than the pimple itself.

For smaller pimples, ingredients such as benzoyl peroxide, salicylic acid, or retinoids may help reduce inflammation more safely. Hydrocolloid pimple patches can also protect the area and reduce the temptation to pick at the skin.

If a pimple is large, painful, persistent, or cystic, it's best to seek professional treatment rather than squeezing it yourself and risking unnecessary scarring.

🎥 In this video, Dr Daniel explains what really happens beneath the skin when you pop a pimple and why protecting the skin barrier is key to preventing acne scars.

Can HIFU Treat Vertical Neck Lines?

Not all neck lines are caused by the same underlying problem—and identifying the cause is essential before choosing a treatment.

Vertical neck bands are primarily caused by contraction of the platysma muscle beneath the skin. Because the underlying problem is muscular, HIFU is generally not an effective treatment. Instead, these bands are more commonly treated by relaxing the muscle with a neuromodulator.

Horizontal neck lines, however, are more closely related to collagen loss and skin laxity. Treatments such as HIFU, radiofrequency microneedling, and ablative radiofrequency can help stimulate collagen and improve the appearance of these lines.

Different types of neck lines have different causes, which is why an accurate diagnosis is essential before selecting the appropriate treatment.

🎥 In this video, Dr Thean explains why HIFU is suitable for some neck concerns but not others, and how identifying the underlying cause guides treatment.

Threading Gone Wrong 😱

Facial threading is a common method of hair removal, but it can occasionally lead to folliculitis—an inflammation of the hair follicles that appears as small red or pus-filled bumps.

This can happen for several reasons:

Repeated friction from the thread can irritate the skin and hair follicles.
Hair removal may trigger inflammation around the follicles, even without an infection.
Bacteria or debris can enter the follicles through the tiny openings created during threading, increasing the risk of folliculitis.
Poor hygiene during the procedure may also contribute to superficial skin infections.

Although folliculitis can resemble acne, it is a different condition and may require a different treatment approach. Mild cases often settle on their own, while more significant reactions may require medical treatment, such as antibiotics.

If you experience persistent or worsening bumps after threading, avoid picking or squeezing them, as this may increase inflammation and the risk of post-inflammatory hyperpigmentation (PIH).

🎥 In this video, Dr Chiam explains why folliculitis can occur after facial threading and how it can be managed safely.

How Did I Manage My Melasma? Part 2

Managing melasma is not only about removing existing pigment. Long-term control also means reducing the processes that continue stimulating pigment production.

Dr Thean's approach focused on several components of melasma:
• Breaking down existing pigmentation
• Reducing inflammation and melanocyte activity
• Addressing abnormal blood vessels
• Supporting the basement membrane and overall skin health

Lifestyle also plays an important role. Sun exposure, stress and insufficient sleep can contribute to melasma activity, which is why consistent sun protection and healthy lifestyle habits remain part of long-term management.

By treating melasma as a chronic condition rather than simply a collection of brown spots, the goal becomes clearer: reduce existing pigment, control ongoing pigment production, and maintain healthier, more stable skin over time.

🎥 In Part 2, Dr Thean shares how addressing multiple components of melasma—and not just the visible pigment—changed the way he managed his own condition.

Vitamin C & Retinol: Can You Use Them Together?
Yes — but they should be used with caution, especially for those with sensitive or reactive skin. While both are evidence-based actives with complementary benefits, combining them incorrectly can increase the risk of irritation.

• Vitamin C: A potent antioxidant that helps brighten the skin, fade pigmentation, stimulate collagen synthesis, and protect against environmental damage.
• Retinol: A form of vitamin A that promotes skin renewal by increasing cell turnover, smoothing texture, reducing fine lines, and improving acne.

In this video, Dr. Daniel shares how to incorporate both ingredients safely — typically by applying Vitamin C in the morning and Retinol at night to maximise efficacy while minimising irritation.

Why Bother Treating Your Pigmentation If You're Always Under The Sun? Part 1

Spending a lot of time outdoors does not mean pigmentation treatment is pointless.

Whether you enjoy golf, running, swimming or simply spend long hours outdoors, pigmentation can still be improved. The goal, however, is to have realistic expectations. If your skin is constantly exposed to UV, achieving complete clearance may not always be possible—but that doesn't mean treatment cannot make a meaningful difference.

For many patients, the aim is to lighten pigmentation, improve overall skin tone, and prevent it from becoming progressively worse, while allowing them to continue enjoying the activities they love.

If you spend a lot of time outdoors, protecting your results becomes just as important as the treatment itself:

Apply a broad-spectrum sunscreen daily
Reapply every 1–2 hours during prolonged sun exposure
Wear physical sun protection, such as hats or visors
Consider oral antioxidants to complement your sun protection strategy

🎥 In Part 1, Dr Chiam explains why an active outdoor lifestyle doesn't mean you have to give up on treating your pigmentation.

How Did I Manage My Melasma? Part 1

Melasma is a chronic and multifactorial skin condition. Treating only the visible brown pigment may lighten the skin temporarily, but it does not necessarily address why new pigment continues to form.

Beyond excess pigmentation, melasma can also involve:
• Chronic inflammation
• Overactive melanocytes
• Abnormal vascular activity
• Changes to the basement membrane

This is why simply breaking down existing pigment may not be enough. While pigment is being cleared, overactive melanocytes may continue producing more, contributing to recurrence.

For Dr Thean, understanding this changed the way he approached his own melasma—from focusing primarily on pigment removal to addressing both existing pigment and the underlying processes driving its production.

🎥 In Part 1, Dr Thean shares his personal experience with melasma and why treating the brown patches alone wasn't enough.

Can Creams Remove Warts?

Warts and skin tags cannot simply disappear within seconds after applying a cream.

Warts are caused by a viral infection within the skin, while skin tags are benign growths made up of connective tissue and blood vessels. Both are actual tissue structures that need to be gradually treated or physically removed.

Some topical treatments can help, but they typically work over days to weeks by gradually breaking down the affected tissue. Products strong enough to destroy a lesion immediately may cause chemical burns, scarring, infection, or pigmentation.

When removal is required, medical options may include:
• Cryotherapy
• Cauterisation
• Laser treatment
• Minor surgical removal

Most importantly, not every skin growth is a wart or skin tag. An accurate diagnosis should come first before attempting removal, particularly before using strong products purchased online.

🎥 In this video, Dr Daniel explains why “instant wart removal” videos can be misleading and how warts and skin tags are safely treated.

Swelling From CO₂ Laser

Some redness and mild swelling can occur after fractional CO₂ laser treatment, but significant or worsening facial swelling is not an expected reaction and should be medically assessed.

Fractional CO₂ laser creates microscopic channels in the skin, temporarily disrupting the skin barrier while the treated areas heal. During this period, the skin can be more vulnerable to irritation and allergens.

Significant swelling after treatment may be related to:
• An excessive inflammatory response
• A reaction to post-treatment skincare or medication
• Contact with an allergen while the skin barrier is compromised
• Treatment intensity that is not appropriate for the individual skin

This is why post-treatment care is especially important after ablative laser procedures. Keep the skin clean, use only recommended skincare, maintain adequate hydration, and minimise unnecessary exposure to potential irritants while the barrier heals.

If swelling becomes significant, continues to worsen, or develops unexpectedly, seek medical assessment rather than trying to treat it yourself.

🎥 In this video, Dr Chiam explains why excessive swelling can occur after CO₂ laser treatment and why proper aftercare matters during recovery.
