Acute Abdomen Pain.

Xanuunka Caloosha ee Degdega ah (Acute Abdomen) waa xaalad degdeg ah oo u baahan qiimeyn caafimaad iyo sawir-qaadis (Ultrasound, X-ray, CT).

Ultrasound Findings:

Qabsinka weyn oo aan la cadaadin karin → Acute Appendicitis.

Xameetida oo gidaarkeedu dhumuc yeeshay + dhagaxyo → Acute Cholecystitis.

Mindhicir ballaaran oo dareere leh → Bowel Obstruction.

Dheecaan xor ah oo caloosha ku jira → Perforation ama dhaawac gudaha ah.

Radiology Findings (X-ray/CT):

Air-fluid levels badan → Bowel Obstruction.

Hawo ka hooseysa diaphragm-ka → Perforated Viscus.

Barar iyo caabuq ku wareegsan appendix → Acute Appendicitis.

Dheecaan ama dhiig gudaha caloosha → Trauma ama ruptured ectopic pregnancy.

# # # **Radiology & Ultrasound Short Note**

**Ultrasound findings:**

* Gallbladder distension (hydropic gallbladder)
* Marked gallbladder wall thickening (>3 mm)
* Pericholecystic fluid
* Sludge in gallbladder lumen (may be present)
* Positive sonographic Murphy sign (pain on probe pressure)
* Absence of gallstones
* Increased wall vascularity on Doppler (sometimes)

**CT findings (done):**
* Gallbladder wall thickening and enhancement
* Pericholecystic fluid or fat stranding
* Distended gallbladder
* No calculus.

# # # **Risk factors:**

* ICU patients
* Sepsis or trauma.
* Prolonged fasting / TPN
* Post-operative state

# # # **Complications:**

* Gangrenous cholecystitis
* Perforation
* Abscess formation.

**Qeexid:**

**Ultrasound (US):**

* Mindhicirro ballaadhan (>2.5–3 cm).
* Dhaqdhaqaaq mindhicireed oo kordhay marxaladda h**e (hyperperistalsis).
* Dareere ku urursan gudaha mindhicirka.
* Mararka qaarkood waxaa la arkaa sababta xannibaadda sida hernia ama buro.

**Raajada (X-ray):**

* Mindhicirro ballaadhan oo hawo iyo dareere leh.
* Heerar badan oo hawo-dareere ah (multiple air-fluid levels).
* Hawo yar ama aan jirin qaybaha hoose ee mindhicirka (distal bowel).

**Gunaanad:**

A hydrocele is an abnormal collection of serous fluid within the tunica vaginalis surrounding the te**is. It is a common cause of painless scrotal swelling in adult males and children.

A hydrocele is an abnormal collection of serous fluid within the tunica vaginalis surrounding the te**is. It is a common cause of painless scrotal swelling in adult males and children.
Radiology (Ultrasound) Findings 📡
Anechoic fluid collection surrounding the te**is within the tunica vaginalis.
Te**is is usually normal in size and echotexture.
Fluid may be simple (clear anechoic) or complex with septations, debris, or echoes in infection or hemorrhage.
Color Doppler shows normal testicular vascularity.
Hydrocele may be unilateral or bilateral; often larger anterior and lateral to the te**is.
Differential Diagnosis
Hematocele – echogenic or complex fluid after trauma.
Pyocele – complex septated fluid with infection.
Spermatocele/Epididymal cyst – cystic lesion arising from epididymis.
Inguinal hernia – bowel loops or omentum within the sc***um.

Acute Appendicitis – Short Radiology Education
Definition:
Acute appendicitis is inflammation of the appendix and is a common cause of acute abdominal pain requiring surgical treatment.

Ultrasound Findings:

Non-compressible blind-ending tubular structure

Appendix diameter > 6 mm

Wall thickening

Increased periappendiceal fat echogenicity

Increased Doppler vascularity

Possible appendicolith

CT Findings:

Enlarged appendix (> 6 mm)

Wall enhancement and thickening

Periappendiceal fat stranding

Appendicolith

Fluid collection or abscess if perforated

Key Point:
A non-compressible appendix measuring more than 6 mm with surrounding inflammatory changes strongly suggests acute appendicitis.

Radiology Pearl:
Ultrasound is preferred in children and pregnant women, while CT provides the highest diagnostic accuracy in adults.

🩺 Gallstones are one of the most common causes of right upper abdominal pain, especially after consuming fatty foods. Ultrasound is the investigation of choice and can easily identify characteristic findings.

✅ Diagnosis: C. Gallstones (Cholelithiasis)

The presence of multiple echogenic foci within the gallbladder producing posterior acoustic shadowing is a classic ultrasound appearance of gallstones.

Ruptured Ectopic Pregnancy – Radiology Short Note
Definition:
Ruptured ectopic pregnancy is a life-threatening obstetric emergency resulting from rupture of an extrauterine pregnancy, most commonly within the fallopian tube, causing hemoperitoneum.

Ultrasound Findings (TVS):

Empty uterine cavity in a patient with positive β-hCG.

Adnexal mass or complex heterogeneous lesion separate from the o***y.

Tubal ring sign may be identified.

Moderate to large free fluid in the pelvis.

Echogenic free fluid (hemoperitoneum), particularly in the pouch of Douglas.

Free fluid in the hepatorenal recess (Morison's pouch) suggests significant intra-abdominal bleeding.

Occasionally, an extrauterine gestational sac with fetal pole and cardiac activity may be visualized.

Key Diagnostic Features:

Positive pregnancy test.

No intrauterine pregnancy.

Adnexal ectopic mass.

Hemoperitoneum.

Pearl

The combination of an empty uterus, adnexal mass, and echogenic free intraperitoneal fluid in a woman of reproductive age with a positive β-hCG should be considered a ruptured ectopic pregnancy until proven otherwise.
