إنجاز طبي في مشفى طرطوس الوطني .. نشر حالة جراحية نوعية في مجلة عالمية محكمة
نشرت واحدة من أرفع المجلات الطبية العالمية المتخصصة تقريراً عن حالة جراحية نادرة ومعقدة أجراها فريق طبي وجراحي في مشفى طرطوس الوطني،
وهي المجلة الدولية لتقارير حالات الجراحة (International Journal of Surgery Case Reports)

وتناول التقرير حالة جراحية معقدة تمثلت ب"إصلاح جراحي بمرحلة واحدة لتضييق برزخ أبهر غير مشخص لدى بالغ مع توسع جذر الأبهر ومجازات إكليلية".
وفيما يلي أبرز ماجاء في التقرير:
"يُشخَّص تضيّق الأبهر (Coarctation of the Aorta – CoA) عادةً في مرحلة الطفولة، إلا أن بعض الحالات قد تبقى غير مشخصة حتى سن البلوغ. وقد يؤدي ظهور المرض في مرحلة متأخرة لدى البالغين إلى تشابه أعراضه مع أمراض القلب والأوعية الدموية المكتسبة، مما قد يسمح بتطور مضاعفات قلبية ثانوية متقدمة.
تسلط هذه الحالة الضوء على صعوبات تشخيص تضيّق الأبهر لدى البالغين والتدبير الجراحي لهذا العرض غير الشائع.
عرض الحالة :
رجل يبلغ من العمر 42 عاماً حضر وهو يعاني من قصور قلب مترقٍ وألم صدري متكرر.
أظهر التقييم الطبي وجود تضيّق شديد في الأبهر لم يكن مشخصاً سابقاً، وكان مترافقاً مع:
"أمّ دم" في الأبهر الصاعد بحجم 5.5 سم، وقلس أبهري شديد (تسرّب شديد في الصمام الأبهري)، وتضيق مهم داخل الدعامة في الشريان الأمامي النازل الأيسر (LAD)، أي عودة التضيق داخل الدعامة السابقة في أحد الشرايين التاجية الرئيسية.
وخضع المريض لعمل جراحي مركب بمرحلة واحدة تضمنت:
1. مجازة إضافية تشريحية (Extra-anatomic bypass) لعلاج تضيّق الأبهر.
2. استبدال جذر الأبهر.
3. مجازة للشريان التاجي (CABG) لإعادة تروية الشريان المتأثر.
وسجلت فترة ما بعد الجراحة تحسن سريري واضح دون مضاعفات تُذكر."
وأشار التقرير إلى أن النقطة الأهم في هذه الحالة ليست مجرد تضيق أبهر بسيط؛ الرجل لديه تضيّق أبهر خلقي بقي غير مكتشف حتى عمر 42 عاماً، ومع مرور السنوات أدى اضطراب جريان الدم وارتفاع الضغط الناتج عنه إلى مجموعة مضاعفات خطيرة: تمدد الأبهر + قصور الصمام الأبهري + مرض تاجي + قصور قلب، لذلك كانت الجراحة المركبة بمرحلة واحدة هي محور التقرير.

Single-stage surgical repair of undiagnosed adult aortic coarctation with concomitant aortic root pathology and coronary artery disease: a case report

Introduction and importance: Coarctation of the aorta (CoA) is usually diagnosed in childhood, although delayed adult presentation may mimic acquired cardiovascular disease and result in advanced secondary cardiac complications. This case highlights the diagnostic challenges and surgical management of this uncommon presentation. Case presentation: A 42-year-old man presented with progressive heart failure and recurrent chest pain. Evaluation revealed previously undiagnosed severe CoA associated with a 5.5-cm ascending aortic aneurysm, severe aortic regurgitation, and significant left anterior descending artery in-stent restenosis. The patient underwent successful single-stage repair comprising extra-anatomic bypass, aortic root replacement, and coronary artery bypass grafting, with an uncomplicated postoperative course
and marked cinical
improvement. Clinical discussion: Adult CoA may remain undiagnosed until secondary cardiovascular complications dominate the clinical presentation. In selected patients requiring concomitant cardiac surgery, a single-stage strategy can provide definitive anatomical and physiological correction while avoiding staged interventions.
Conclusion: This case highlights the significance of considering adult CoA in patients with unexplained complicated cardiovas- cular disease. Early recognition and appropriate patient selection allow effective single-stage surgical intervention in meticulously selected instances.

Although usually diagnosed in childhood, some cases remain undetected until adult: hood, resulting in severe morbidity and premature mortalityl341 Adults' presentation is often nonspecific, manifesting as hyperten- sion, exertional dyspnea, or heart failure symptoms?2), A systolic blood pressure gradient of 220 mmHg between the upper and lower extremities remains a crucial diagnostic ndicator!1,s1 Echocardiography and computed tomography angiography are essential for diagnosis and anatomical assessmentlea, Although endovascular treatment is recommended for specific isolated cases, surgical repair remains necessary in patients with underlying
Adult aortic coarctation may go untreated and manifest as secondary cardiovascular problems, including severe heart failure. • A progressive pathophysiological cascade that includes hypertension, aortic aneurysm, valve dysfunction, and coronary artery disease can be triggered by undetected congenital lesions. . A complete clinical examination is still essential for diag- nosis, especially if there is a difference in blood pressure between the upper and lower limbs. In complicated instances, a single-stage surgical repair that combines extra-anatomic bypass, aortic root replacement, and cor- onary revascularization is possible. • While avoiding phased procedures, comprehensive surgi- cal correction can produce positive hemodynamic and clinical results.
