Coronary artery disease (CAD) is one of the most common cardiovascular conditions. In complex cases with severe coronary artery calcification, extensive calcium buildup can make arteries rigid and difficult to expand. This can make stent placement challenging or unfeasible, potentially compromising long-term treatment outcomes and increasing the risk of restenosis.

However, Intravascular Lithotripsy (IVL) can help treat heavily calcified coronary arteries. Using a specialized balloon catheter, IVL delivers low-energy shockwaves that create small fractures in calcified plaque, reducing its rigidity and improving vessel compliance. This facilitates balloon angioplasty and stent placement, when appropriate.

IVL is another treatment option for patients with coronary artery disease and complex lesions, severe calcification, multi-segment stenosis, or challenging vessel preparation before stent placement.

Feeling tired while speaking is a red flag for pulmonary edema in heart failure.
Increasing shortness of breath in patients with heart failure should not be dismissed as ordinary tiredness. It may be a warning sign of fluid retention, which can eventually lead to fluid buildup in the lungs and progressively worsening breathing difficulties.

Dr. Piyanart Preeyanont, an interventional cardiologist at MedPark Hospital, recommends watching for these 5 important red flags that may signal an increased risk of pulmonary edema in patients with heart failure:

🚩 Increasing shortness of breath with exertion: Activities you used to do comfortably, such as walking, bathing, or climbing stairs, may suddenly leave you unusually breathless.
🚩 Shortness of breath while speaking or eating: Difficulty breathing during everyday activities may indicate that symptoms are beginning to interfere with daily life.
🚩 Difficulty breathing when lying flat: Needing more pillows, sleeping upright, or waking up at night gasping for air may signal worsening fluid retention.
🚩 Unexplained cough: Particularly at night, when lying flat, or accompanied by an unusual increase in phlegm.
🚩 Swelling of the legs, feet, or ankles: A sign that excess fluid may be accumulating in the body, often accompanied by rapid weight gain.
“Don’t wait until symptoms become severe… and you’re struggling to catch your breath.”

If your shortness of breath noticeably worsens, seek medical evaluation promptly so your treatment plan can be adjusted. Never increase diuretics or adjust medication doses on your own.

Warning: If you experience severe shortness of breath, inability to speak in full sentences, chest pain, lightheadedness, or decreased consciousness, call 1669 or go to the emergency room immediately.

Recognizing these warning signs early is key to preventing heart failure from worsening and progressing to an emergency.

Heart failure does not involve only the heart. Many patients, particularly older adults, usually have comorbidities, such as diabetes, hypertension, kidney disease, thyroid disorders, nutritional problems, and impaired physical function. Managing only some of these conditions may lead to suboptimal disease control and increase the risk of severe exacerbations.

'Lutetium-177 PSMA: A Treatment Option for Metastatic Prostate Cancer

An emerging medical innovation, Lutetium-177 PSMA therapy, is a targeted radiopharmaceutical therapy for metastatic or hormone-resistant prostate cancer.

• Targeted delivery: Intravenous Lu-177 binds to PSMA, highly expressed on prostate cancer cells.
• Selective tumor destruction: Emits beta radiation to destroy cancer cells while sparing surrounding tissue, reducing side effects.
• Indication: For patients with metastatic castration-resistant prostate cancer (mCRPC) with PSMA-positive lesions on PSMA PET/CT.

Treatment is administered under a nuclear medicine specialist, with pre-treatment evaluation including complete blood count, liver and kidney function, and PSA. It is typically given every 6 weeks for up to 6 cycles, depending on clinical response and the doctor’s discretion.
