🇹🇭Wanna be… : คนไข้ฉีดฟิลเลอร์ใต้ตามาเเล้ว …เวลายิ้มขึ้นก้อน ทำยังไงดี🔥

📖คนไข้มาด้วยปัญหากังวลใจเรื่องของ ”ก้อนใต้ตา“ โดยเฉพาะเวลายิ้ม ซึ่งคนไข้เคยได้รับการฉีดfillerใต้ตามา 2 ครั้งครับ ครั้งล่าสุดมากกว่า 1 ปีเเล้ว ➡️ ครั้งเเรกคนไข้เเจ้งว่าฉีดเเล้วไม่เต็ม ผลลัพธ์ไม่ค่อยชัด ➡️ จึงตัดสินใจไปฉีดเพิ่มครั้งที่ 2 พบว่า overfilled เเละเป็นก้อน ➡️ จากนั้นจึงตัดสินใจไปฉีดสลาย ✅ ได้ผลลัพธ์สุดท้าย ดูได้จากภาพ before
🥇ครั้งเเรกที่ผมเห็นรูปคนไข้ตอนยิ้มก็คิดว่าfillerเก่าต้องเหลือเยอะเเน่ๆเลย เพราะ lump ชัดมาก…ก็เลยขอรูป baseline ก่อนทำหัตถการใดๆเลยมาดูครับ พบว่าคนไข้มี OOM โดยเฉพาะ orbital part ที่มี hypercontraction อยู่เเล้วเป็น baseline ด้วยส่วนนึง (เเต่ปฏิเสธิไม่ได้ว่า หลังฉีด HA filler ไปเเล้ว lump ตรง Tear trough(Tt) ใหญ่ขึ้นจริง)
🔑ดังนั้น…เมื่อใดก็ตามที่คนไข้ที่มีปัญหาลักษณะนี้ ได้รับการเเก้ไขปัญหาใต้ตาด้วย การฉีดฟิลเลอร์ตรง Tt supraperiosteum plane ซึ่งเป็น conventional technique ➡️ ก็จะ always lumps อย่างที่เห็นจากในรูปครับ (lumpมากหรือน้อย เร็วหรือช้าขึ้นอยู่กับปริมาณเเละเนื้อ filler ที่ฉีด)
🌟ตอนนี้ผมมี 2 ทางเลือก ⬇️⬇️⬇️
✅ ฉีดสลายเเล้วค่อยฉีดใหม่
✅ ฉีดเเก้ไขไปเลยโดยไม่ต้องสลาย
📚ถึงจุดนี้…เเต่ละท่านเลือกchoiceไหนให้คนไข้กันบ้างครับ ?
🔑โดยส่วนตัว ในเคสนี้ผมเลือก… ✅ ฉีดเเก้ไขไปเลยโดยไม่ต้องสลาย
เนื่องจากผมลองตรวจด้วย ultrasound ดูเเล้วพบว่ามี HA filler กระจายอยู่ใน OOM จริง…ทั้ง2ข้างปริมาณเเละความหนาตัวไม่เท่ากัน โดยข้างซ้ายของคนไข้มี ความหนาของ HA ที่มากกว่า…เเต่อย่างไรก็ตาม HA นั้นอยู่ต่ำลงมา กว่า ส่วนของ Tt ซึ่งใกล้กับ lower end of OOM(orbital part) ➡️ ซึ่งคาดว่าเพราะได้รับการฉีดสลาย ตรง Tt มาบ้างเเล้ว…เเละความหนาเเละclusterของ HA filler ที่เห็นจาก USG ไม่สามารถอธิบาย lump ที่เราเห็นทั้งหมดได้ครับ⬇️⬇️⬇️
เนื่องจากคนไข้มี 🔑key problem หลักอื่นๆอีกที่มี effect ให้เกิน OOM hypercontraction เเละ over-compensate
✅ Lateral face volume loss (muscle ทาง medial side จะ compensate เเทน muscle ทาง lateral part ที่สูญเสีย balance จากเรื่องของ Length and tension relationship)
✅ ซักประวัติเพิ่มเติมเเละตรวจร่างกาย ควบคู่กับการUSG พบว่ามี filler ที่ร่องเเก้มเยอะมาก ซึ่งทำให้ ส่วน insertion point of lip levator muscle ไม่สามารถทำงานได้อย่างที่ควรจะเป็น เลยต้องอาศัย OOM มาช่วยเเทน (จะเห็นว่า eye opening ของคนไข้ at before เล็กกว่าหลังจากที่ผมทำการเเก้ไขไปแล้ว at after)
✅ filler บางส่วนที่ยังหลงเหลืออยู่ในบริเวณของ Tt เเละ orbital part of OOM
🌟วิธีเเละเทคนิคที่ผมเลือกมาเเก้ไขในเคสนี้คือ⬇️
✅ Correct lateral face volume loss ด้วย HA filler ประมาณ 1.5-2.0 cc (รวม2ข้าง)
✅ Correct volume loss at premaxillary space (เฉพาะ lower half) 0.3-0.5 ml/side
✅ Correct deep plane at submalar area 0.2 ml at lt.side
✅✅✅plan เสริม Toxin เข้าไปด้วยครับที่ : โดยเฉพาะ OOM(orbital part) + LLSAN + Nasalis (transverse part)
🔥ทั้งหมดนี้ใช้HA fillerเพียงเเค่ 3 cc ครับ
🇹🇭ผลลัพธ์ที่ได้ :
✅ midface ดู harmonize ขึ้นอย่างชัดเจน
✅ lump ที่เคยเห็นชัด…ดูดีขึ้นถึง 80% (โดยไม่ได้ฉีดสลายเพิ่ม)
✅ Nasojugal groove ดีขึ้นอย่างชัดเจน
✅ eye openning ดูดีขึ้น
✅ periorbital wrinkle ทั้ง lateral side เเละ medial side ดูดีขึ้น 40-50% โดยยังไม่ได้ฉีดtoxinใดๆ
✅ lower half of midface เเละ lower face ที่ดู saggy ก็ดูดีขึ้นเช่นกันครับ
🔑สรุป สำหรับผมเอง การฉีดฟิลเลอร์จุดใดๆก็ตาม จะมีผลต่อจุดรอบๆเสมอทั้ง static and dynamic เเม้ไม่ได้ฉีด…ดังนั้น anatomical knowledge เเละ ความ precise ของเทคนิคการฉีด มีผลมากกว่าที่เราจะนึกออกครับ🙏🏻🥹🥰

🇹🇭 Wanna be… : Case sharing 🔥
“3 cc with myomodulation and applying anatomical knowledge to real‑world aesthetic practice”
📖This patient presented with the main concern of “under‑eye lumps,” especially visible on smiling. She had previously received two sessions of tear trough filler (the last one more than one year ago). After the first treatment, she felt the result was insufficient and not very noticeable, so she decided to have a second session, which resulted in overfilling and visible lumps. She then underwent filler dissolution, as you can see in the before images.
🥇My first impression, when I saw her smiling photo, I thought there must be a significant amount of residual filler, because the lumps were very obvious. So I asked for her true baseline photo, before any procedures had been performed. I found that she already had hypercontraction of the orbicularis oculi muscle (OOM), particularly the orbital part, as part of her baseline. Of course, after HA filler was injected, the lump in the tear trough (Tt) area became more prominent.
🔑Therefore, in patients with this kind of issue, when the under‑eye area is corrected with conventional tear trough supraperiosteal filler injection alone, you will almost always see lumps like in the photo. The severity and onset of lump formation depend on the total volume and rheology of the filler product.
🌟 At this point, I had two options:
✅ Dissolve and then reinject later
✅ Correct the problem directly without dissolving
📚 In your own practice, which option would you choose for this type of patient?
🔑 Personally, I chose: ✅ Direct correction without dissolving.
Ultrasound examination showed that there was HA filler distributed within the OOM on both sides, with asymmetry in thickness and amount. The left side had thicker HA. However, the residual HA was located inferiorly than the tear trough , closer to the lower end of the orbital part of the OOM, probably because some filler at the Tt had already been dissolved. The thickness and clustering pattern of HA seen on ultrasound did not fully explain the visible lump.
🔥The key was that this patient had several other underlying problems contributing to OOM hypercontraction and over‑compensation:
✅ Lateral facial volume loss: The medial muscles were compensating for the lateral muscle imbalance due to changes in the length–tension relationship.
✅ Further history and physical examination, together with ultrasound, revealed a large amount of filler in the nasolabial fold area. This compromised the normal function of the lip levator muscle at its insertion point, forcing the OOM to over-compesation. You can see that the patient’s eye opening in the before photo is smaller than after correction.
✅ Residual filler at the tear trough and within the orbital portion of the OOM.
🌟 My strategy and techniques for this case were:
✅ Correct lateral facial volume loss with HA filler, approximately 1.5–2.0 cc in total (both sides).
✅ Correct volume loss in the premaxillary space (lower half only) with 0.3–0.5 ml per side.
✅ Correct the deep plane in the submalar area with 0.2 ml on the left side.
✅✅✅ I also planned to add toxin in: OOM (orbital part), LLSAN, and the transverse part of the nasalis muscle.
🔥 The total HA filler used in this case was only 3 cc.
🇹🇭 Outcomes:
✅ The midface looks clearly more harmonized.
✅ The previously obvious lump improved by about 80%, without additional dissolution.
✅ The nasojugal groove shows marked improvement.
✅ Eye opening is better.
✅ Periorbital wrinkles, both lateral and medial, improved by around 40–50%, even before toxin injection.
✅ The saggy lower half of the midface and lower face also improved.
🔑 In summary, for me, any filler injection point will influence surrounding structures, both in static and dynamic conditions, even in areas we did not inject directly. That is why detailed anatomical knowledge and precise injection technique have far more impact than we might initially imagine. 🙏🏻🥹🥰
