ภาวะปัสสาวะคั่งหลังได้รับการระงับความรู้สึกเฉพาะส่วนโดยการฉีดยาชาเข้าช่องน้ำไขสันหลัง (Postoperative urinary retention after spinal anesthesia)
Postoperative urinary retention (POUR) after spinal anesthesia คือ ภาวะที่ผู้ป่วยไม่สามารถปัสสาวะเองหลังได้รับการระงับความรู้สึกเฉพาะส่วนโดยการฉีดยาชาเข้าช่องน้ำไขสันหลัง (spinal anesthesia) แม้กระเพาะปัสสาวะจะเต็ม (full bladder) และ ผู้ป่วยไม่มีปัญหาทางระบบปัสสาวะมาก่อน โดยพิจารณาจากอาการปวดหรือไม่สบายท้อง ผู้ป่วยไม่ปัสสาวะภายในเวลา 6-8 ชั่วโมงหลังการผ่าตัด หรือ ตรวจพบปริมาณปัสสาวะในกระเพาะปัสสาวะ > 400-600 mL โดยการทำ bladder ultrasound ซึ่งคำนวณได้ 2 แบบ
1. Bladder volume = Width x Depth x Height x 0.7
2. Largest transverse diameter > 10.7 cm แปลผลว่ามี full bladder
หากผู้ป่วยไม่ได้รับการวินิจฉัยและรักษาในระยะเวลาที่เหมาะสมอาจทำให้เกิดภาวะแทรกซ้อน ได้แก่ อาการปวดท้องน้อยรุนแรง ภาวะ bladder overdistension หากเกิดนานมากกว่า 2-3 ชั่วโมงอาจทำความเสียหายต่อกล้ามเนื้อกระเพาะปัสสาวะ (detrusor muscle) และเส้นประสาท ทำให้กระเพาะปัสสาวะทำงานผิดปกติ (bladder dysfunction) แบบถาวร ภาวะเพ้อสับสน (delirium) ในผู้สูงอายุ เพิ่มระยะเวลาการนอนโรงพยาบาล ภาวะแทรกซ้อนจากการใส่สายสวนปัสสาวะ เช่น การติดเชื้อในทางเดินปัสสาวะ การบาดเจ็บของท่อปัสสาวะ (urethral injury)
กลไกการปัสสาวะ(micturition mechanism) แบ่งเป็น 2 ระยะ ได้แก่ ระยะเก็บปัสสาวะ(storage phase) และ ระยะปล่อยปัสสาวะ (voiding phase) ซึ่งอยู่ภายใต้การควบคุมของ somatic และ automatic nervous system
1. Storage phase ถูกควบคุมโดยระบบประสาท sympathetic (T11-L2) จะกระตุ้นให้ detrusor muscleคลายตัว และ internal urethral sphincter หดตัว ระบบประสาท somatic (pudendal nerve, S2-S4) กระตุ้นให้ external urethral sphincter หดตัว ส่งผลให้เก็บปัสสาวะได้อย่างมีประสิทธิภาพโดยไม่รู้สึกปวดหรือรั่ว
2. Voiding phase เมื่อมีปริมาณปัสสาวะ 300-500 mL จะกระตุ้น stretch receptors ที่ผนังกระเพาะปัสสาวะผ่าน afferent nerves (pelvic nerve, S2-S4) ไปpontine micturition center (PMC) ซึ่งจะกระตุ้นระบบประสาท parasympathetic (S2-S4) ทำให้ detrusor muscle หดตัว ยับยั้งsympathetic (T11-L2) ทำให้ internal urethral sphincter คลายตัว และ ยับยั้ง pudendal nerve (S2-S4) ทำให้ external urethral sphincter คลายตัว โดยมี cerebral cortex ควบคุม voluntary เพื่อตัดสินใจว่าจะปัสสาวะหรือกลั้นปัสสาวะต่อ
spinal anesthesia บล็อค sacral segments (S2-S4) มีผลต่อ voiding phase โดยยับยั้งทั้ง afferent nerves ทำให้สูญเสียความรู้สึกของ bladder fullness และ ยับยั้ง efferent parasympathetic ทำให้เกิดdetrusor areflexia, urethral sphincter dysfunction เกิดภาวะ urinary retention โดยปกติ bladder function จะ recovery ช้ากว่า sensory/motor มักต้องรอให้ยาชาหมดฤทธิ์ถึงระดับS2-4ถึงปัสสาวะได้ปกติ
Incidenceของ POUR after spinal anesthesia พบได้ 5%-70% ซึ่งอุบัติการณ์ที่หลากหลายเป็นผลจากความเสี่ยงของผู้ป่วยในการเกิดภาวะ POUR ในแต่ละการศึกษามีความแตกต่างกันมาก โดยปัจจัยเสี่ยงให้เกิด POUR after spinal anesthesia แบ่งออกเป็น 3 ด้าน
1.ปัจจัยด้านผู้ป่วย (Patient-related) ผู้ป่วยที่มีความเสี่ยงต่อการเกิด POUR ได้แก่
- เพศชาย
- อายุ > 50 ปี
- โรคประจำตัว (comorbidities): Benign prostatic hyperplasia (BPH), renal disease และ โรคอื่นๆ เช่น stroke, poliomyelitis, cerebral palsy, multiple sclerosis, spinal lesions, diabetic neuropathy, alcoholic neuropathy
2.ปัจจัยเสี่ยงที่เกี่ยวข้องกับทางวิสัญญี (Anesthetic-related)
เทคนิคการระงับความรู้สึก spinal anesthesia เพิ่มความเสี่ยงของ POUR มากกว่า epidural anesthesia และ มากกว่า general anesthesia ตามลำดับ
สำหรับ spinal anesthesia การใช้ยาชาขนาดสูงเพิ่มความเสี่ยงต่อ POUR แบบ dose dependent และ ยาชาที่ระยะเวลาออกฤทธิ์ยาวจะยิ่งบล็อก sacral segments (S2-S4) นานขึ้นเพิ่มความเสี่ยงของ POUR โดยถ้าใช้ยาชาในปริมาณที่เท่ากัน เรียงลำดับยาชาที่มีความเสี่ยงให้เกิด POUR จากมากไปน้อย ดังนี้ bupivacaine > ropivacaine > mepivacaine > lidocaine > prilocaine > 2-chloroprocaine ในต่างประเทศนิยมใช้ 2-chloropocaine (short-acting), prilocaine (intermediate-acting) ในambulatory/day case surgery แต่ยังไม่มีในประเทศไทย ปัจจุบันมี bupivacaine (long-acting) แนะนำให้ใช้ในขนาดที่เหมาะสมเพียงพอสำหรับการผ่าตัด และ ควรเฝ้าระวังในผู้ป่วยที่เสี่ยงสูงต่อการเกิด POUR สำหรับยาชาที่ออกฤทธิ์แบบ intermediate-acting เช่น lidocaine และ mepivacaine ไม่ควรใช้เนื่องจากมี neurotoxicityเสี่ยงต่อการเกิด transient neurological symptoms (TNS)
intrathecal opioid ทำให้เกิดurinary retention จากหลายกลไก
1 กลไกหลักเกิดจากยา opioid จับกับ µ-receptor ที่sacral spinal cord (S2-S4) ส่งผลยับยั้งparasympathetic outflow ลดการหดตัวของ detrusor muscle ลด afferent sensation จากstretch receptors ทำให้ไม่รู้สึกปวดปัสสาวะ
2 Rostral spreading ยา opioid ลอยตาม CSF ขึ้นด้านบนไปจับกับ opioid receptor ที่ pontine micturition center (PMC) รบกวนการทำงานของ micturition reflex กลไกนี้มีความสำคัญในmorphine ซึ่งเป็น hydrophilic opioid
3 ยา opioid diffusion สู่ epidural space และ ถูกดูดซึมเข้าสู่กระแสเลือดเกิด secondary systemic opioid effect
คุณสมบัติในการจับกับไขมันของยา opioids ที่ฉีดเข้าintrathecal space มีผลต่อ POUR โดย hydrophilic opioid (morphine) เกิด urinary retention มากกว่า lipophilic opioid (fentanyl) เนื่องจาก morphine ออกฤทธิ์นานกว่า และ จับกับทั้ง spinal & supraspinal opioid receptors ขนาดของยา intrathecal opioid ที่สูงมีผลต่อการเกิด urinary retention มากกว่าขนาดยาที่ต่ำแบบ dose-dependent มีการศึกษาพบว่า spinal morphine เพิ่มความเสี่ยงต่อการเกิด urinary retention ค่อนข้างสูงแม้ใช้ morphine ในขนาดที่ต่ำ ส่วน spinal fentanyl เป็น adjuvant ที่ดีสำหรับ a**lgesia ระหว่างผ่าตัดและหลังผ่าตัดระยะสั้น โดยไม่เพิ่มความเสี่ยง POUR อย่างมีนัยสำคัญเมื่อใช้ในขนาดต่ำ (ไม่ควรใช้เกิน 25 mcg) ร่วมกับยาชาที่ออกฤทธิ์ไม่นาน (prilocaine หรือ low-dose bupivacaine)
คุณสมบัติในการจับกับ receptor ของยา opioids มีผลต่อ POUR โดยยากลุ่ม opioid full agonists (fentanyl, morphine, sufentanil) มีผลต่อการเกิด urinary retention มากกว่า opioid partial agonists (nalbuphine, buprenorphine) เนื่องจาก opioid agonists จับกับ µ-receptor ได้ดีกว่า และ ไม่มี ceiling effect มีการศึกษาเพื่อเปรียบเทียบประสิทธิภาพในการระงับปวดหลังผ่าตัดคลอด และ ผลข้างเคียงระหว่าง spinal nalbuphine (0.8mg) กับ spinal morphine (0.2mg) ผลการศึกษาพบว่า กลุ่ม nalbuphine ให้การระงับปวดที่ดีกว่า และ มีผลข้างเคียง pruritus และ urinary retention น้อยกว่ากลุ่ม morphine แม้ nalbuphine จะมีข้อดีเรื่องการลดผลข้างเคียง โดยเฉพาะ pruritus และ urinary retention แต่ spinal morphine ยังคงเป็นตัวเลือกหลักในแนวปฏิบัติสากล เนื่องจาก morphine ให้การระงับปวดที่นานและลึก มีการศึกษาจำนวนมากสนับสนุนว่า morphine ให้การระงับปวดดีกว่า วิสัญญีแพทย์ส่วนใหญ่คุ้นเคยกับ morphine มากกว่าและสามารถจัดการผลข้างเคียงของ morphine ด้วยการใช้ยา (ondansetron หรือ low-dose naloxone) การทำ bladder scan + intermittent catheterization ทำให้ปัจจุบัน intrathecal morphine เป็น gold standard สำหรับระงับปวดหลังการผ่าตัดคลอด ส่วน nalbuphine แม้จะให้การระงับปวดได้ดี แต่โดยรวมยังด้อยกว่า morphine ในหลายการศึกษา ปัจจุบัน nalbuphine เป็น alternative สำหรับผู้ป่วยที่เสี่ยงผลข้างเคียงจาก opioids สูง
3.ปัจจัยเสี่ยงที่เกี่ยวข้องกับการผ่าตัด (Surgery-related)
การผ่าตัดบางประเภทมีผลรบกวนกลไกการปัสสาวะ (micturition mechanism) เพิ่มความเสี่ยงต่อการเกิด postoperative urinary retention (POUR) โดยมีการรายงานอุบัติการณ์ดังนี้
ประเภทการผ่าตัด และ Incidence ของ POUR (โดยประมาณ)
- Anorectal surgery (hemorrhoidectomy, fistulectomy) 1-52%
- Joint arthroplasty (hip/knee replacement) 10-84%
- Inguinal hernia repair 5.9-38%
- Spine surgery 9-26%
- Pelvic organ prolapse (POP) repair 25-31%
- General gynecologic surgery 16%
อย่างไรก็ตาม ในการณีการผ่าตัดที่มีความเสี่ยงสูงต่อ POUR เช่น การผ่าตัดทางระบบทางเดินปัสสาวะ (urological surgery) ผู้ป่วยมักได้รับการใส่สายสวนปัสสาวะตั้งแต่ช่วงการผ่าตัดจนถึงหลังการผ่าตัดเพื่อป้องกันภาวะแทรกซ้อน
ระยะเวลาการผ่าตัดที่นานขึ้นเกี่ยวข้องกับปัจจัยที่เพิ่มความเสี่ยงต่อการเกิด POUR ได้แก่
- ปริมาณสารน้ำทางหลอดเลือดดำ (IV fluid) ที่ได้รับเพิ่มขึ้น ส่งผลให้กระเพาะปัสสาวะเต็มเร็วและเกิด overdistension มีการศึกษาพบว่าผู้ป่วยที่เข้ารับการผ่าตัด inguinal hernia repair และ a**l surgery โดยวิธี spinal anesthesia การให้ IV fluid > 750 mL ในระหว่างการผ่าตัดจะเพิ่มความเสี่ยงต่อ POUR มากขึ้น 2.3 เท่า
- ระยะเวลาการระงับความรู้สึกที่ยาวนานขึ้น โดยเฉพาะในผู้ป่วยที่ได้รับ spinal anesthesia ซึ่งทำให้สูญเสียความรู้สึก (sensation) ของกระเพาะปัสสาวะ
- การเคลื่อนไหวที่ลดลง (immobility)
- การใช้ opioid ระหว่างการผ่าตัดและหลังการผ่าตัดในปริมาณที่มากขึ้น
โดยทั่วไปการผ่าตัดที่ใช้เวลานานกว่า 2–4 ชั่วโมงจะเพิ่มความเสี่ยงของ POUR อย่างมีนัยสำคัญ การให้ผู้ป่วย early ambulation ภายใน 24 ชั่วโมงหลังการผ่าตัดจะลดความเสี่ยงต่อ urinary retention ได้
การป้องกันและการรักษาภาวะ POUR after spinal anesthesia
ภาวะ POUR ไม่ใช่ major complication ที่ทำให้เกิดอันตรายร้ายแรงถึงชีวิต แต่ทำให้เกิดความไม่สุขสบายและเกิดผลแทรกซ้อนอื่นๆ ผู้ป่วยที่ได้รับการระงับความรู้สึกด้วยวิธี spinal anesthesia ควรได้รับการประเมินความเสี่ยงต่อการเกิด POUR โดยวิสัญญีแพทย์ตระหนักถึงความเสี่ยงที่ควคุมไม่ได้ และปรับลดหรือหลีกเลี่ยงความเสี่ยงที่สามารถควบคุมแก้ไขได้
การให้การระงับปวดแบบ multimodal a**lgesia เพื่อหลีกเลี่ยงหรือลดการใช้ยา systemic opioids และการกระตุ้นให้ผู้ป่วย early ambulation ช่วยลดความเสี่ยง urinary retention หลังการผ่าตัดได้ กรณีที่ผู้ป่วยมีความเสี่ยงต่อ POUR ต่ำ และ ได้รับ intrathecal lipophilic opioid ในขนาดไม่สูง ผู้ป่วยน่าจะสามารถปัสสาวะได้เอง ยังไม่จำเป็นต้องสวนปัสสาวะ (intermittent bladder catheterization) อย่างไรก็ตามควรติดตามอาการจนผู้ป่วยสามารถปัสสาวะได้ ในผู้ป่วยที่มีความเสี่ยงต่อ POUR สูง ควรติดตามอาการผู้ป่วยหลังการผ่าตัดอย่างใกล้ชิด หากมีอาการปวดหรือไม่สบายท้อง หรือ ผู้ป่วยไม่สามารถปัสสาวะเองภายในเวลา 6 ชั่วโมงหลังการผ่าตัด หรือ bladder ultrasound scan พบปริมาณปัสสาวะในกระเพาะปัสสาวะ > 400-600 mL ควรรักษาโดยการสวนปัสสาวะและติดตามอาการจนผู้ป่วยสามารถปัสสาวะได้เอง
