Evidence based physiotherapy (physical therapy) specialised in pain and injury management, and restoration of movements, strength, and physical wellbeing due to various acute and chronic conditions. Physiotherapy [or physical therapy in the US and few other countries], is a specialty in health care that can help you restore or improve your ability to move because of an injury or a health problem.

Physiotherapy is concerned with assessing and maximizing quality of life and movement potential within the spheres of promotion, prevention, treatment/intervention, and rehabilitation. It involves the interaction between physical therapist (PT), patients/clients, other health professionals, families, care givers, and communities in a process where movement potential is assessed and goals are agreed upon, using knowledge and skills unique to physiotherapists. Physios use an individual's history and carry out physical assessments to arrive at a diagnosis and establish a management plan and, when necessary, incorporate the results of laboratory and imaging studies. Physiotherapy has many specialties including cardiopulmonary, neurologic, orthopaedic, sports, geriatrics and pediatrics, to name some of the more common areas.

🦵 “Not every ACL tear automatically needs surgery.”

A growing body of evidence continues to challenge the idea that ACL reconstruction must always be the first-line pathway.

A recent paper published in the British Journal of Sports Medicine reported that around 2 in 3 patients were able to avoid ACL reconstruction and still achieve satisfactory outcomes through well-structured non-operative management.

This does NOT mean surgery has no role.

ACL reconstruction remains an important and appropriate option for many individuals, especially depending on instability, goals, sport demands, associated injuries, and personal context.

But perhaps the conversation is becoming more nuanced:

💡 The question may no longer be:
“Physiotherapy OR surgery?”

But rather:
• Who is the right patient for each pathway?
• What are the patient’s goals?
• When is the best timing?
• And how can rehabilitation be optimised regardless of the decision?

For clinicians, this reinforces the importance of:
✅ Shared decision-making
✅ Individualised care
✅ Evidence-informed rehabilitation
✅ Avoiding one-size-fits-all narratives

Because successful ACL management is often less about defending a single approach…
and more about matching the right strategy to the right person.

We often tell patients:

“Do your exercises -they’ll help you heal.”

But how?

This paper gives a powerful answer:

👉 Load changes tissue.

Through a process called Mechanotransduction, mechanical stress is converted into cellular activity that drives repair and remodelling. 

In simple terms:

Movement → cellular signalling → protein synthesis → tissue adaptation

The authors even reintroduce the term “mechanotherapy” - using exercise as a treatment to stimulate healing, not just improve fitness. 

And importantly:

• Muscle responds to load by activating growth pathways

• Controlled loading improves alignment and regeneration

• Too little load leads to tissue loss, while appropriate load cause stronger, better tissue

Which raises a key clinical question:

Are we prescribing exercise as a habit…
or as a targeted biological stimulus?

Exercise isn’t just something we give patients…

It’s one of the most powerful treatment tools we have. 💪🧠
