🧠 Stroke Rehabilitation 2026: Physical Activity และ Gait Training หลัง Stroke
จาก 2026 Guideline for Adult Stroke Rehabilitation and Recovery การฟื้นฟูหลัง stroke ไม่ควรหยุดเพียงแค่ “เดินได้” แต่ควรมุ่งให้ผู้ป่วย เดินได้ดีขึ้น มีกิจกรรมทางกาย และกลับไปใช้ชีวิตจริงได้อย่างต่อเนื่อง
❤️ Physical Activity
ผู้ป่วย stroke ควรได้รับ individualized exercise program เพื่อเพิ่ม cardiorespiratory fitness และ mobility รวมถึงลดความเสี่ยง vascular events ในอนาคต (COR 1, LOE A)
เป้าหมายการออกกำลังกาย ได้แก่
• Moderate-intensity physical activity 150–300 นาที/สัปดาห์
• Aerobic training 3–5 วัน/สัปดาห์
• Strength training 2–3 วัน/สัปดาห์
• Balance training 2–3 วัน/สัปดาห์
การออกกำลังกายควรเริ่มตั้งแต่ช่วง rehabilitation และทำต่อเนื่องเมื่อกลับบ้านหรือเข้าสู่ชุมชน โดยปรับโปรแกรมให้เหมาะสมและสามารถทำได้จริงในผู้ป่วยแต่ละคน
🚶 Gait & Mobility: หัวใจคือ Task-specific training
Guideline แนะนำ task-specific training (COR 1, LOE A) โดยเน้น
👉 Overground walking + Stepping + Balance
คือ ถ้าเป้าหมายคือ “อยากให้เดินดีขึ้น” ผู้ป่วยต้องได้ ฝึกเดินจริง ด้วยกิจกรรมที่ใกล้เคียงกับการใช้งานจริงและมีการฝึกซ้ำอย่างเพียงพอ
นอกจากนี้ยังมีวิธีที่อาจช่วยเสริม gait rehabilitation ได้แก่
• Moderate- to high-intensity stepping/locomotor training
• Resistance training ทั้ง affected และ less-affected leg
• Rhythmic auditory cueing
• Lower-limb CIMT
• Motor imagery / mirror therapy
• Interactive gait training เช่น VR/exergames
โดยวิธีเหล่านี้ส่วนใหญ่อยู่ในระดับ COR 2a
ส่วน FES ร่วมกับ exercise, aquatic exercise, TENS, visual biofeedback, acupuncture ร่วมกับ rehabilitation และการรักษา spasticity ร่วมกับ gait training อยู่ในระดับ COR 2b — may be considered
⚠️ อะไรไม่แนะนำเพื่อเพิ่ม walking/mobility ในผู้ป่วยที่เดินได้แล้ว?
❌ Robot-assisted gait training (RAGT) ไม่แนะนำแทน task-specific overground gait training — COR 3: No Benefit, LOE A
❌ Light- to moderate-intensity treadmill training ไม่ว่าจะมีหรือไม่มี body-weight support ไม่แนะนำแทน task-specific overground gait training — COR 3: No Benefit, LOE A
❌ Neurodevelopmental treatment เช่น Bobath ไม่แนะนำเพื่อเพิ่ม walking และ mobility — COR 3: No Benefit, LOE B-R
ทั้งนี้ guideline ระบุว่า RAGT และ treadmill-based interventions อาจให้ผลใกล้เคียงกับ overground task-specific training แต่มีประเด็นด้านค่าใช้จ่ายและ accessibility
🔬 Emerging / Under Investigation
แนวทางที่ยังมีหลักฐานจำกัดและต้องศึกษาเพิ่มเติม ได้แก่
• Split-belt treadmill
• Noninvasive neuromodulation
• Tai chi / Qigong
• Ischemic conditioning
• Multimodal interventions เช่น neuromodulation + robotics + VR
สำหรับ high-frequency rTMS ที่ ipsilesional hemisphere ร่วมกับ gait training มี preliminary evidence ว่าอาจช่วยเพิ่ม walking speed แต่ผลต่อ mobility โดยรวมยังไม่ชัดเจนและต้องศึกษาเพิ่มเติม โดย guideline ระบุว่า neuromodulatory techniques ส่วนใหญ่ยังถือเป็น investigational devices
📌 สิ่งสำคัญอีกเรื่องคือ Intensity และ Dose
Intensity = ฝึกหนักแค่ไหน
ควรให้ cardiovascular/locomotor intensity เพียงพอและเหมาะสมกับผู้ป่วย
Dose = ฝึกมากแค่ไหนโดยรวม
ครอบคลุมทั้ง duration, frequency, repetitions และระยะเวลาของโปรแกรม
ดังนั้นหัวใจของ gait rehabilitation อาจจำง่าย ๆ ว่า
👉 Task-specific practice + Sufficient intensity + High repetition + Adequate training dose
เป้าหมายสุดท้ายไม่ใช่เพียง “เดินได้ในห้องฝึก” แต่คือการนำความสามารถนั้นไปสู่ การมีกิจกรรมทางกายและการเคลื่อนไหวอย่างต่อเนื่องในชีวิตจริง
