One of the most misunderstood parts of a consultation is this:

A patient asks for one procedure… and the surgeon recommends something else.

That does not mean the patient was “wrong.”
It simply means the visible concern and the correct solution are not always the same thing.

A patient may think the issue is fat, when the real issue is loose skin.
Another may ask for implants, when the breast position shows that a lift is more important.
Someone else may request a facelift, when the change they are noticing is mostly related to volume loss.

This is exactly why consultations matter.

The surgeon’s role is not to confirm a procedure name.

It is to understand what is creating the concern, what can improve it safely, and what type of result is realistic.

Sometimes that means confirming the treatment you asked for.
Sometimes it means changing the plan.
And sometimes it means explaining why surgery may not be the right step at all.

A good recommendation should be based on your anatomy not just your request.

Where do we get cartilage for rhinoplasty when there isn’t enough usable septal cartilage?

In many rhinoplasty cases, the septum provides enough cartilage to support the changes we need to make.

But if the septal cartilage is severely deviated, damaged, or simply not sufficient, we may need another source.

Depending on the patient and the surgical plan, cartilage can sometimes be taken from the ear or the ribs to provide the additional support needed.

The important part is choosing the right option for the individual patient and the structural needs of the nose.

Why do breast implants sometimes look high or too firm right after surgery?

Because the result you see in the first few weeks is not the final result.

After breast augmentation, the tissues are still swollen and the muscles and skin need time to adjust to the implant. This can make the breasts look higher, firmer or slightly more “boxy” at first.

As the swelling decreases and the tissues relax, the implants gradually settle into a more natural position.

This settling process often starts becoming noticeable within the first few months, but the breasts can continue changing and softening for 6 to 12 months.

So if your breasts look unusually high immediately after surgery, it doesn’t necessarily mean something is wrong — healing takes time.
