TUBAL REVERSAL SURGERY
Reopening Previously Tied Fallopian Tubes
Today, we performed tubal reversal surgery for a patient whose fallopian tubes had previously been tied for permanent contraception and who now wished to have another child.
Tubal reversal, also known as tubal reanastomosis, aims to reconnect the healthy remaining segments of the fallopian tubes using microsurgical principles and restore natural tubal passage.
For appropriately selected patients, tubal reversal can be an alternative to IVF. One of its potential advantages is that, if tubal function is successfully restored, the patient may have repeated opportunities to conceive naturally rather than relying on a single assisted-reproduction cycle.
Published studies report pregnancy rates after tubal reversal of approximately 42–69%, while pooled series cited by the American Society for Reproductive Medicine have reported pregnancy rates of around 65–68% in selected patients.
The likelihood of success depends on several factors, including the patient’s age, ovarian reserve, the original sterilization method, the length and condition of the remaining tubes, pelvic adhesions, endometriosis, and the partner’s s***m parameters.
It is also important to know that the risk of ectopic pregnancy is higher after tubal reversal, with rates of approximately 4–8% reported in the literature. Therefore, early ultrasound evaluation is recommended once pregnancy is confirmed.
With careful patient selection and appropriate microsurgical technique, tubal reversal may offer some women another opportunity for natural conception.
