Sağlıklı günler diliyorum, Alt yüz bölgesindeki yaşlanma kriterlerinden bahsetmek istiyorum sizlere. Alt yüz bölgesini oluşturan yapılar arasında çene ve boyun kısmı bunların en önemlisidir. Yaşlanmayla birlikte çenenin ucunda aşağıya doğru bir inme ortaya çıkabiliyor. Bu inmenin nedeni; birlikte içindeki kemiklerin küçülmesi, bağların zayıflaması, kasların gevşemesi ve çene ucundaki yağ dokusunun erimesidir. Yanağın özellikle yan kısımlarında buradaki anatomik değişiklikler ve derin plandaki yapıların gevşemesiyle yanakların aşağıya sarkması özellikle yaşlılığın kriterlerinden bir diğeridir. Boyun bölgesinde platisma bantları dediğimiz boynun deri altındaki platisma kasının gevşemesi ve burada bantların ortaya çıkması da diğer bir yaşlanma kriteri olarak karşımıza çıkmaktadır.⁣⁣

Aging Face Signs: Lower Face Area⁣⁣

I would like to tell you about the aging criteria in the lower face area. Among the structures that compose the lower face region, the chin and neck are the most important of them. Downward drooping can occur at the tip of the chin with aging. The reasons for this drooping of the structures are that; the shrinkage of the bones, weakening of the ligaments, loosening of the muscles and atrophy of the fat tissue at the tip of the jaw. One of the other criterion of lower facial aging is that the drooping at the the cheek region. This complaints form especially in the lateral parts of the cheek, due to the anatomical changes of the tissue and the loosening of the structures in the deep plane. The other aging criterion is the laxity of the platysma muscle under the skin of the neck and the appearance of the bands here, which we call as platysma bands in the neck region.⁣⁣

Radyoterapi sonrası Kapsül Kontraktürü İçin Yağ Enjeksiyonu⁣⁣

İmplant ile meme onarımlarından sonra radyoterapi görmesi gereken hastalarımızın bir kısmında radyoterapinin etkilerine bağlı olarak memede kapsül kontraktürü ve fibrozis adını verdiğimiz bir rahatsızlık ortaya çıkıyor. Genellikle radyoterapi yani ışın tedavisinden 6 ay sonra başlayan ve 2 yıla kadar devam eden bu kapsül kontraktürü ve memede kasılma hali, yağ enjeksiyonları ile yumuşatma ve düzeltme gerektirebiliyor. Özellikle kapsül kontraktürü gelişen, kol hareketleri ile memede ağrı olan, memede sertleşme ve ciddi derecede kasılma ortaya çıkan hastalarımızda yağ enjeksiyonlarını sıklıkla tercih ediyoruz. Bu yağ enjeksiyonları ne şekilde gerçekleştiriliyor? Kapsül kontraktürü nedeniyle yapılan yağ enjeksiyonunun ayrıntılarını yarından itibaren sizlere açıklayacağım.⁣⁣

Fat Injection For Breast Capsule Contracture Due to Radiotherapy⁣⁣

In some of our patients who need to receive radiotherapy after breast reconstruction with implants, capsule contracture and fibrosis occur in the breast due to the effects of radiotherapy. This capsule contracture, which usually starts 6 months after radiotherapy and continues for up to 2 years, may require softening and correction with fat injections. We often prefer fat injections especially for our patients who have capsule contracture, pain in the breast with arm movements, fibrosis and contraction of the breast. Parsiyel Meme Defektlerinin Onarımında Yağ Enjeksiyonu Uygulaması-4⁣⁣

Kısmi yani parsiyel meme defektlerinin onarımında kullanılan yağ enjeksiyonlarındaki uyguladığımız stratejilerden bahsetmek istiyorum. Bu hastalarımızda doku defektinin miktarına göre yağ enjeksiyonlarını seanslara bölmekteyiz. Yani, tek bir aşamada oradaki defekti bütünüyle tamamlamak amacıyla hareket etmiyoruz. Daha önceki yağ enjeksiyonu videolarımda bu konudan bahsetmiştim. Yağ bolüs şeklinde yani toplu olarak bir noktaya verildiği zaman yağın orta kısmındaki yağ hücrelerinin öldüğünden ve bunların kalsifikasyon adını verdiğimiz sertleşmelere gittiğinden bahsetmiştim. Bu nedenle, bu yağları dokuların arasına tabaka tabaka yerleştirerek bölgede belirli bir büyüme elde ediyoruz. 6 ay sonra ikinci bir seansta orada biraz daha büyüme elde ediyoruz. Bu şekilde, seanslar halinde bu bölgedeki defekti onarmaktayız.⁣⁣

Fat Injection For Partial Breast Defects Reconstruction-4⁣⁣

I would like to talk about the strategies that we apply in fat injections used in partial breast defects reconstruction. In these patients, we separate the fat injections into sessions according to the amount of tissue defect. That is, we do not act to complete the whole defect totally in a single step. I had talked about this issue in my previous fat injection videos. I had mentioned that when the fat is injected as a bolus (i.e. given to a single point in high volume), the fat cells in the middle part are going to be necrosis. Anymore, stiffening can be formed which we term as calcification. For this reason, we achieve a certain expansion in the defect through placing these fats layer by layer between the tissues. We obtain more development with a second session after 6 months. By this way, partial breast defect can be reconstructed in several sessions.⁣⁣

Parsiyel Meme Defektlerinin Onarımında Yağ Enjeksiyonu Uygulaması-3⁣⁣

Memede ortaya çıkabilen diğer hastalıklara bağlı bazı defektlerin de düzeltilmesinde yağ enjeksiyonu kullanılabilmektedir. Bunlardan bazıları memenin selim huylu tümörlerinden ortaya çıkan ve o bölgenin çıkarılmasına bağlı oluşabilen defektlerin düzeltilmesi amacıyla uygulanmaktadır. Bunun yanında memedeki bazı iltihabi hastalıklarda, granülomatöz mastit dediğimiz bazı hastalıklarda memenin o kısmının alınmasının gerektirdiği hallerde de kullanılmaktadır. Ayrıca, memede ortaya çıkabilen yanıklar ve travmalara bağlı doku kayıplarından sonra her iki memenin asimetrik durumunun düzeltilmesi için de yağ enjeksiyonları sıklıkla kullanılabilmektedir. Bunun yanında doğuştan olan meme eksikliklerinde ve gelişim bozukluklarında bazı tüberöz meme hastalarında öncelikli olarak yağ enjeksiyonu yaparak doku kazanımı sağlıyoruz. Bunda sonra da silikon protezler ile büyütme yapabilmekteyiz.⁣⁣

Fat injection can be used to correct some defects due to other diseases that may occur in the breast. Some of these are applied to correct the defects that arise from benign tumors of the breast and may occur due to the removal of that region. In addition, it is also used in some inflammatory diseases of the breast, in some granulomatous mastitis diseases when it is necessary to remove that part of the breast. Also, fat injections can often be used to correct the asymmetrical condition of both breasts after tissue loss due to burns and traumas that may occur in the breast. Furthermore, we primarily provide tissue gain by injecting fat for patients who have developmental disorders and for some tuberous breast patients who have congenital breast deficiencies. After that, we can make augmentation with silicone prostheses.⁣⁣

Parsiyel Meme Defektlerinin Onarımında Yağ Enjeksiyonu Uygulaması-2⁣⁣

Sizlere meme rekonstrüksiyonlarında parsiyel meme defektinin onarılması için yağ enjeksiyonu kullanımından bahsetmiştim. Lokal olarak alınmış olan meme kanserlerinden sonra ortaya çıkan çukurluklar, eksiklikler ve defektlerin giderilmesi için yağ enjeksiyonu uygulaması sıklıkla kullanılabilmektedir. Bu işlem hemen eş zamanlı olarak yapılmıyor. Ne zaman yapılıyor? Tümör çıkarıldıktan sonra çeşitli radyoterapi ve kemoterapi işlemleri bittikten en az 6 ay sonra bu uygulamalara başlayabilmekteyiz. 6 aydan itibaren memeye bir seans yağ enjeksiyonu yapıyoruz. Bu defektin onarım miktarına bakıyoruz. Bundan 6 ay sonra ikinci bir seans yaparak bu bölgenin tamamen onarılmasını sağlayabilmekteyiz. Bu defektler farklı nedenlerden de ortaya çıkabilmektedir. I had told you about the use of fat injection for partial breast defect reconstruction. Fat injection can be used frequently to eliminate the protrusions, deficiencies and defects that occur after breast cancers. This process is not applied simultaneously. When is it possible to be performed? After the tumor is removed, we can start these process at least 6 months later when various radiotherapy and chemotherapy procedures are completed. We can make one session of fat injection to the breast beginning from 6 months after surgeries. We are making some control for the reconstruction ratio of this defect. After 6 months, we can perform a second session and ensure that this area is completely reconstructed. These defects can occur for different reasons. I will tell you about these reasons tomorrow.⁣⁣

Meme Onarımı Sonrası Memeye Yağ Enjeksiyonu-2⁣⁣

Meme onarımları sonrasında memedeki düzensizliklerin ve şekil bozukluklarının düzeltilmesinde yağ enjeksiyonlarının kullanıldığından bahsetmiştim. Bunları ne şekilde kullanıyoruz? Mastektomi dediğimiz memenin içinin boşaltılması işlemi yapılırken burada deri altındaki meme dokusunun düzensizliğinden dolayı bazı bölümlerde daha fazla deri altına giriliyor, bazı bölümlerde daha yüksek kalabiliyor. İçine implant koyulduğu zaman dışarıdan bakıldığında yer yer çukurluklar ve çıkıntılar oluşabiliyor. Mastektomi ve onarım sürecinden sonra aşağı yukarı en erken altıncı aydan itibaren vücuttan alınanan yağların bu bölgelere enjekte edilerek, bu düzensizliklerin giderilmesi sağlanmaktadır. Bu şekilde, makyaj prosedürü adını verdiğimiz onarım sonrası dönemde bu düzensizlikleri giderebilmekteyiz. Bu işlemleri birkaç seansa da yayarak daha iyi bir hale getirebilmekteyiz.⁣⁣

Fat Injection To The Breast After Breast Reconstruction-2⁣⁣

I had mentioned that fat injections are used to correct irregularities and deformities on the breast after breast reconstructions. How do we use them? While the breast tissue is removed with subcutaneous mastectomy procedures, it may be possible to form some irregularity and indentation on the breast due to the subcutaneous breast tissue interference. When the implant is placed inside for reconstructive purpose, depressions and protrusions may seen in different part of the breast surface from external look. Six month is the earliest time to perform autologus harvested fat injections following mastectomy. The fat can be harvested from the body and injected into these undulate areas to eliminate this tissue defect. By this way, we can correct these irregularities in the post-reconstruction period which we call as post-op make-up procedure. We can perform several session of fat injection to improve the recent result.⁣⁣

Memeye yapılan yağ enjeksiyonularını iki büyük başlık altında toplamıştık. Bunlardan bir tanesi estetik meme büyütme amaçlı yağ enjeksiyonları, diğeri de meme onarımından sonra memede düzeltme amaçlı yapılan yağ enjeksiyonlarıdır. Estetik meme yağ enjeksiyonlarından sizlere bahsetmiştim. Şimdi de onarım amaçlı yağ enjeksiyonlarını ne şekilde yapıyoruz, bu konuda bilgi vermek istiyorum. Herhangi bir nedenden dolayı (meme kanserinden veya doğuştan olabilir) memede bozukluklar görülebiliyor. Memeye yağ enjeksiyonunu memeye yeni bir görüntü verme, iki taraf arasındaki asimetriyi dengelemek amaçlı kullanabilmekteyiz. We have gathered fat injections to the breast under two big headings. One of them is applied for aesthetic breast augmentation, and the other is using for correction the breast deformity following breast reconstruction. I had told you about aesthetic breast fat injections. Now, I would like to give information about how we perform fat injections for reconstructive purposes. Breast deformity can be seen for any reason (it can be from breast cancer or congenital). We can use fat injection to reform the breast in a new look and to balance the asymmetry between the two sides. In addition, breast capsular contracture may occur in patients undergoing radiotherapy. We can reduce this radiotherapy damage and reverse its effects with fat injection. I will explain you in detail from tomorrow, how the breast fat injections are used for different purposes.⁣⁣

Yağ enjeksiyonu ile meme büyütme işlemi sağlıklı bir yöntem mi? şeklinde sorular tarafımıza soruluyor. En merak edilen konulardan bir tanesi de bu. Memeye verilen yağın yağ kistleri oluşturabileceğini, ilerde radyolojik görüntülerde karışıklık olabileceğini sizlere anlatmıştım. Ancak daha da önemli bir konu yağın memeye verildiği zaman bir problem teşkil edip etmeyeceğidir. Bu konuda Dünya’da bir fikir birliği yok. Bu konuyla ilgili yüzlerce araştırma yapılıyor. Bazıları olumlu, bazıları olumsuz çıkabiliyor. Ancak burada çok önemli bir nokta var. Meme dokusundan ne kadar uzağa yağ enjeksiyonu yapılırsa, o kadar güvenli bir şekilde memeye yağ enjeksiyonu işlemi elde edilmektedir. Burada hastalarımızdan özellikle memeye yağ enjeksiyonunda bir onay istiyoruz. Bu ameliyatla ilgili sorumluluğu üstümüze alıyoruz diye. Is Fat Injection To The Breast A Healthy Procedure?⁣⁣

We received questions about whether breast augmentation with fat injection is a healthy method or not? This is one of the most frequently asked questions. I have told you that the fat given to the breast can form sebaceous cysts and that there may be confusion in the radiological images in the future. However, an even more important issue is whether the fat will create a problem when it is transfered into the breast. There is no consensus in the world on this issue. Hundreds of studies have been done on this topic. Some may be positive, some may be negative. However, there is a very important point here. How far fat is injected from the breast tissue, safer procedure will be achieve. We want approval for consent form from our patients who take responsibility for this surgery, especially in fat injection into the breast. Due to this situation is not fully clarified in the world, we cannot inform the patient for certain result.⁣⁣

Memeye Yağ Enjeksiyonu Prensipleri-3⁣⁣

Memeye yapılan yağ enjeksiyonlarında çok dikkatli bir uygulama yapma gerekliliğinden bahsetmiştim. Eğer fazla miktarda yağ dokusu memenin içine enjekte edilirse o bölgede yağ hücrelerinin ölebileceğini söylemiştim. Bunlar memede farklı hastalıklarla karışabilmektedir. Memede özellikle mamografi ve ultrason yapıldığı zaman, yağ enjeksiyonu sonrası ölen hücrelerin kireçlenmesi sonrasında burada kistlerin oluşması ve başka hastalıklarla karışması mümkün olmaktadır. Burada bir mamografi fotoğrafı görüyorsunuz. Yağ hücrelerinin memenin içinde kistik ve kalsifiye dediğimiz kireçlenmiş durumlarını görüyorsunuz. Bu kireçlenmeler farklı meme hastalıkları ve meme kanserleriyle karışabilmektedir. Bunlar kansere sebep olmasa bile mamografi sonuçlarında karışıklık ortaya çıkabilmektedir. Bundan dolayı meme dokusunun içine yapılmaması ve doğru teknikle uygulanması gerekmektedir.⁣⁣

Principles Of Fat Injection To The Breast-3⁣⁣

I had mentioned the necessity of performing a very careful application in fat injections to the breast. I had said that if a large amount of fat tissue is injected into the breast, the fat cells in that area may die. These can be confused with different diseases in the breast. Especially, when mammography and ultrasound are performed in the breast, it is possible to form cysts after calcification of cells that die after fat injection and intervene with other diseases. Here, you see a photo of a mammogram. You see the cystic and calcified state of the fat cells in the breast. These calcifications can be confused with different breast diseases and breast cancers. Even if these do not cause cancer, confusion may occur in mammography results. Therefore, it should not be applied inside the breast tissue and should be applied with the correct technique.⁣⁣
