A dorsal hump is not always the primary problem. Sometimes, it’s only the consequence of a deeper imbalance. Before removing anything, identify what is really distorting the profile.

This isn’t a hump problem—it’s a balance problem. A high radix and an underprojected tip create the illusion of a larger hump. Correct the imbalance first, then decide how much dorsal reduction is actually needed.

Balance begins at the radix. In this case, controlled radix reduction was performed using our previously published Radix Saw technique. For the surgical details, please refer to the original publication.

One of the most overlooked details in rhinoplasty is the facet.

In thick-skinned patients, especially when performing reduction rhinoplasty, a depressed facet should never be ignored. During healing, postoperative soft tissue contraction may accentuate the deformity, making it more noticeable over time.

In selected cases, a facet graft restores the contour, supports the soft tissue envelope, and helps maintain long-term stability.

As with every graft in rhinoplasty, the indication is determined by the anatomy—not by routine.

A hanging alar border should not be confused with an alar notch. It can be encountered in both primary and revision rhinoplasty. Misinterpreting the deformity as an alar notch may lead to unnecessary alar rim grafting and compromise the natural contour of the alar rim.

Correct diagnosis is the key. A precise, conservative marginal trim restores the alar border while preserving its natural contour.

No graft. No complex sutures. A precise marginal trim heals predictably with an inconspicuous scar when performed conservatively.

Nasal harmony is achieved when every small detail is respected.

Some books never grow old. Although this book was published over 30 years ago, it became one of my guides when I first read it about a decade ago, and it still has a place on my desk today.

Years later, I had the privilege of writing my own book.
