Nefes alamamak, hayata aktif katılamamak ve her an nefesin tamamen kesilmesiyle ölüm korkusu yaşamak… Bunlar, videodaki hastamız gibi, iki taraflı ses teli felci olan hastaların genellikle yaşadığı durumlar.

Lazer ile nefes açma ameliyatı sonrası, alınan ilk uykuda bile hasta nefes rahatlığını hissediyor ve hastamın tabiriyle, rahat nefes alabildiği ikinci hayatına başlıyor.

🇬🇧Patients with bilateral vocal cord paralysis are, perhaps, among the patients I’ve seen who truly suffer the most.

Not being able to breathe, not being able to actively participate in life, and living with the fear of death at any moment when breathing is completely cut off… These are the conditions that patients with bilateral vocal cord paralysis, like the one in this video, generally experience.

After laser breathing-opening surgery, even in the first sleep afterward, the patient feels relief in breathing — and, in my patient’s own words, begins a second life in which she can finally breathe freely.

Sürekli boğazınızı temizleme ihtiyacı hissediyor musunuz? 🤔

Gün içinde tekrar tekrar boğaz temizliyor, ama bir türlü rahatlayamıyorsanız… Boğazınızda takılma, gıcık veya yumru hissi varsa… Bunun sebebi “gizli reflü” olarak da bilinen laringofarengeal reflü (LFR) olabilir.

Çoğumuz reflü deyince mide yanması ve ekşimeyi düşünürüz. Ama LFR’de durum farklı: mide içeriği yemek borusunu aşıp boğaz ve gırtlak bölgesine kadar ulaşabiliyor — çoğu zaman hiç mide yanması olmadan. Bu yüzden pek çok hasta, reflü hastası olduğunun farkına bile varamıyor.

Dikkat edilmesi gereken belirtiler:
🔸 Sürekli boğaz temizleme ihtiyacı
🔸 Boğazda takılma ya da yumru hissi
🔸 Geçmeyen gıcık hissi
🔸 Özellikle sabahları belirginleşen ses kısıklığı

🇬🇧 Do you constantly feel the need to clear your throat? 🤔

If you find yourself clearing your throat again and again throughout the day, but never quite get relief… If you have a feeling of something stuck in your throat, irritation, or a lump sensation… The cause might be laryngopharyngeal reflux (LPR), also known as “silent reflux.”

🇬🇧 Our patient had been uncomfortable with his own voice for years — a thin, fragile voice, a condition we call mutational falsetto. It happens when the voice doesn’t deepen enough during puberty, and most people just get used to living with it.

Reinke ödemi, özellikle uzun süredir sigara kullanan hastalarımızda sıkça karşılaştığımız bir ses teli rahatsızlığı. Ses tellerinin hemen altında jöle kıvamında bir sıvı birikiyor ve bu da sesin kalınlaşmasına, kısılmasına, bazen de nefes almada zorluğa yol açabiliyor.

Bu videoda paylaştığım hastamda da benzer bir tablo vardı. Mikrolaringoskopi altında, soğuk alet tekniğiyle ses tellerini koruyarak birikmiş ödemi temizledik ve ses tellerinin ince, titreşebilen yapısını yeniden kazandırdık. Soğuk alet kullanımı, doku üzerinde ısı hasarı oluşturmadığı için iyileşme sürecini de destekliyor.

Ameliyat sonrası doğru ses dinlenmesi ve takip süreciyle hastalarımız genellikle çok daha net ve rahat bir sese kavuşuyor.

🇬🇧 Reinke’s edema is a vocal cord condition we see quite often, especially in patients with a long history of smoking. A jelly-like fluid builds up just beneath the vocal cords, which can make the voice thicker, hoarser, and sometimes even affect breathing.

This patient had a similar picture. Under microlaryngoscopy, using the cold instrument technique, we cleared the accumulated edema fluid while protecting the vocal cords, restoring their thin, vibrating structure. Since cold instruments don’t cause thermal damage to the tissue, this also supports a smoother healing process.

With proper voice rest and follow-up after surgery, our patients usually end up with a much clearer, more comfortable voice.

Bazı ameliyatlar var ki, ameliyathaneden çıkarken içiniz rahatlıyor. Bu da onlardan biriydi.

Hastamızın durumu kolay değildi — çift taraflı ses teli felci zemininde posterior glottik stenoz da eklenince işler bir hayli karmaşıklaşıyor. Normalde bu tip vakalarda uyguladığımız parsiyel aritenoidektomi + J kordotomi ameliyatı, tek başına ses teli felcinde daha yüksek başarı sağlarken, stenoz eklendiğinde başarı oranı düşüyor.

En güzel anı ise ameliyatın sonunda yaşadık: hastamızın trakeostomisini hemen o gün, ameliyat masasından kalkmadan çektik. Yani hasta, boğazında hiçbir tüp olmadan, kendi hava yoluyla ayağa kalktı.

Her vaka bir hikaye, her sonuç bir emek.

🇬🇧 Some surgeries are the kind where you walk out of the OR feeling truly relieved. This was one of them.

Our patient’s case wasn’t an easy one — bilateral vocal cord paralysis combined with posterior glottic stenosis makes things considerably more complex. The procedure we typically perform in these cases, partial arytenoidectomy + J-cordotomy, has a higher success rate on its own in vocal cord paralysis, but when stenosis is added to the picture, the success rate drops.

The best moment came at the very end of the surgery: we removed the patient’s tracheostomy that same day, right there on the operating table. In other words, the patient stood up with their own airway — no tube in their throat.

Every case is a story, every outcome is the result of hard work.

🔴 Boğazınızda sürekli bir yumru hissi var ama yemek yerken rahatsızlık vermiyor mu? Buna “globus” diyoruz ve sandığınızdan çok daha sık görülüyor.

✨ Genellikle ciddi değildir. Reflü, boğaz kaslarındaki gerginlik ve stres rol oynayabilir — kişiden kişiye değişir.

⚠️ Şikayet devam ediyor, artıyor ya da yutma güçlüğü, kilo kaybı, ağrı veya ses kısıklığı eşlik ediyorsa doktora başvurun.

💬 İyi haber: doğru bilgilendirme bile çoğu zaman şikayetler azaltıyor. Yeterli su tüketimi, boğaz temizlemekten kaçınma, kafein ve sigarayı azaltmak da fayda sağlıyor.

🇬🇧 In this video, the two different voices you’ll hear belong to the same person. The change you hear was achieved solely through proper voice therapy.

Mutational falsetto is a voice disorder that occurs when the voice continues to be used in a high-pitched manner, particularly after puberty. With accurate diagnosis and appropriate voice therapy, most patients can achieve a natural, deeper voice without needing surgery.

The statements you hear in the video are our patient’s own experience, and the voice change was achieved entirely through voice therapy.

Bu hastamız, sesinde yaşadığı ani kesilmeler nedeniyle birden fazla KBB hekimine başvurmuş, yapılan muayenelerde “ses tellerin tamamen normal” denilmiş.

🇬🇧 This patient came to us after seeing several ENT doctors for sudden voice breaks — and every time, the exam came back “completely normal.”

And honestly, that’s expected. In spasmodic dysphonia, the problem isn’t the structure of the vocal cords — it’s involuntary spasms in the muscles that control them. When the cords are examined at rest, everything looks fine.

The real diagnosis comes from catching the voice breaks and the muscle spasms happening AT THE SAME TIME, while the patient is actually talking.

“Ses kısıklığı kendiliğinden geçer”,
“fısıldamak sesi dinlendirir”,
“boğaz temizlemek iyi gelir” veya “ses ameliyatı sesi mutlaka bozar” gibi düşünceler, bazen doğru tanı ve tedaviye ulaşmayı geciktirebilir.

Oysa ses kısıklığı, bazen basit bir ses yorgunluğunun belirtisi olabilirken bazen de erken tanı gerektiren önemli hastalıkların habercisi olabilir.

🇬🇧 In this article published in Hürriyet Ege Health last month, I aimed to address some of the most common misconceptions about voice health and share the facts behind them.

Beliefs such as “Hoarseness will go away on its own,” “Whispering helps the voice rest,”
“Clearing your throat is beneficial,” or
“Voice surgery inevitably damages the voice” can sometimes delay proper diagnosis and treatment.

In reality, hoarseness may be a simple sign of vocal fatigue, but it can also be an early warning sign of more serious conditions that require timely evaluation.

If your voice is trying to tell you something, don’t ignore it.
