1. Delaying going to a toilet Keeping your urine in
your bladder for
too long is a bad idea. A full bladder can cause
bladder damage.
The urine that stays in the bladder multiplies

urinary tract
infections, and then nephritis, and even uremia.

sodas and soft drinks. It raises your blood
pressure and your
kidneys start suffering. So you should cut down
the amount of
coke you drink daily.
5. Not drinking water. Our kidneys should be
hydrated properly to
perform their functions well. If we don’t drink
enough, the toxins
can start accumulating in the blood, as there
isn’t enough fluid to
drain them through the kidneys. Drink more than
10 glasses of
water daily. There is an easy way to check if you
are drinking
enough water: look at the colour of your urine;
the lighter the
colour, the better.

PUD ( PEPTIC ULCER DISEASE)

Peptic ulcer disease is the term used to describe inflammation of the gastric or duodenal mucosa.

In the normal stomach, gastric acid (hydrochloric acid) is produced by the parietal cells in the gastric pits, along with digestive enzymes produced by the Chief cells. The lining of the stomach is protected from the gastric acid by a layer of mucous produced by mucous cells. The duodenum, that part of the intestine immediately following the stomach, is also protected by mucous producing cells.

CAUSES
Two of the main causes of peptic ulcer disease are infection with Helicobacter pylori and ingestion of non-steroidal anti-inflammatories (NSAID's).

1. H pylori infection damages the intestinal mucosa and disturbs the balance between acid production and protection and repair. This causes acid destruction of tissue and ulcer formation.

2. NSAID's interfere with blood flow in the mucosal layer and the production of bicarbonate (to neutralise acid) and mucous. This allows acid to pe*****te the mucous layer and attack underlying tissue resulting in ulcer formation.

Peptic ulcers may result in gastric bleeding or may occasionally perforate resulting in peritonitis or sepsis. Ulcers in the duodenum/stomach junction may cause obstruction.

SIGNS & SYMPTOMS
Most patients present with epigastric (upper abdominal) pain described as sharp, dull, burning, or penetrating and may radiate to back.

Ulcer-related pain generally occurs 1-4 hours after meals and often awakens the patient at night. This pattern is believed to be the result of increased gastric acid secretion, which occurs after meals and during the late night and early morning hours. About 50-80% of patients with duodenal ulcer experience nightly pain, as opposed to only 30-40% of patients with gastric ulcer.

Pain is often relieved by food, a finding often cited as being specific for duodenal ulcer.

The pain of duodenal ulcer is generally episodic; however, the pain can evolve into a chronic, daily occurrence in some patients.

A change in the patient's usual pattern of ulcer pain should be considered serious because it may herald an imminent complication. When food or antacids fail to relieve the pain, a high index of suspicion is warranted.

Concern is especially warranted in the setting of new-onset nausea and vomiting, decreased appetite, and weight loss.

Many patients experience a feeling of hunger, and about 20-40% of patients describe bloating, belching, or symptoms suggestive of reflux.

Gastrointestinal bleeding is the most common complications of peptic ulcer. Sudden loss of blood due to erosion of blood vessels is a potentially serious situation. Patients with bleeding ulcers may have a history of bloody vomiting, tar like faeces, episodes of syncope (dizzy or fainting spells) and shortness of breath.

Perforation may result in spillage of stomach contents into the abdominal cavity resulting in peritonitis or sepsis.

Ulcers in the duodenal/stomach junction may cause obstruction. Patients who develop gastric outlet obstruction as a result of chronic, untreated duodenal ulcer usually report a history of fullness and bloating associated with nausea and emesis occurring several hours after food intake.

• Cholecystitis

• Cholangitis

• Pancreatitis

• Pancreatic cancer

• Gastric cancer

• Diverticulitis

• Gastroenteritis

• Inflammatory bowel disease
• Myocardial infarct

INVESTIGATIONS
- Blood for FBC and H pylori serology.

- Faecal H pylori antigen test.

- Urea breath test.

- Endoscopic biopsy for histology, culture and rapid urease test.

TREATMENT
Treatment consist of stopping identifiable causes such as NSAID's or alcohol, reducing acid production with proton pump inhibitors (PPI) and eliminating H pylori with antibiotics if suspected or identified.

Recommended therapy for H pylori infections consists of two or three antibiotics with a proton pump inhibitor.

Triple therapy

Clarithromycin (Biaxin): 500 mg PO bid with Amoxicillin (Amoxil): 1 g PO bid or Metronidazole (Flagyl) 500mg PO bid

And

Omeprazole (Prilosec): 20 mg PO bid or

Lansoprazole (Prevacid): 30 mg PO bid or

Rabeprazole (Aciphex): 20 mg PO bid or

Esomeprazole (Nexium): 40 mg PO qd

Quadruple therapy

Bismuth 525 mg PO qid and Metronidazole 500 mg PO qid and Tetracycline 500 mg PO qid

with

Omeprazole (Prilosec): 20 mg PO bid or

Lansoprazole (Prevacid): 30 mg PO bid or

Rabeprazole (Aciphex): 20 mg PO bid or

Esomeprazole (Nexium): 40 mg PO qd

Quadruple therapy is usually reserved for patients who have failed one course of treatment

Surgery may be required for perforation, obstruction, uncontrolled or recurrent bleeding and symptoms that do not respond to drug therapy.

PATIENT MANAGEMENT
Avoid smoking, alcohol and caffeine as they stimulate gastric acid production.

Avoid aspirin and NSAID's.

Use antacids or histamine blockers to alleviate symptoms.

Avoid foods that aggravate symptoms.
