If your digestion has started acting like it belongs to someone else, I want you to know something: you're not imagining it, and it's probably not the food.
Bloating after meals that never used to bother you. Constipation that comes out of nowhere. A stomach that suddenly reacts differently than it has for decades. These are some of the most common things I hear from women in their 40s and 50s — and almost universally, they come in having already cut out a list of foods and still not feeling better.
Here's what's actually happening: your gut is full of estrogen receptors. As estrogen shifts in perimenopause, your gut motility changes, your gut lining becomes more reactive, and your gut microbiome shifts with it. This isn't necessarily a food problem And the approach that actually works is very different from another elimination diet.
A 2025 survey of nearly 600 women (presented at The Menopause Society's Annual Meeting) found that 94% reported digestive symptoms — and 82% said their symptoms started or got noticeably worse at perimenopause or menopause. This is close to universal, and most women are navigating it with no real explanation and no real plan.
