Colorado Visiting Nurse Association (CVNA) offers At Home Foot Care for Kaiser Permanente Medicare members who are homebound and have a physician referral for home foot care.

A Medical Foot Nurse comes to you, providing services that include a comprehensive foot exam, neuropathy screening, circulation checks, blood pressure monitoring, medication review, and assessment of chronic conditions such as diabetes, heart disease, hypertension, and arthritis. The visit also includes individual health coaching and education.

What happens when we start with what an older adult wants to accomplish in their own home?

CAPABLE takes a client-directed approach, bringing together occupational therapy, nursing, and home-modification support to help older adults achieve their goals for safer, more independent living at home.

In this interview with The Home Accessibility Therapist, Ally Evelyn-Gustave, CAPABLE Clinical Training Specialist, shares her nearly two decades of experience supporting CAPABLE implementation and clinician training. Ally explains why individualized home assessments matter, how OTs can look beyond one-size-fits-all accessibility standards, and why listening to the client is at the heart of meaningful change.

We’re thrilled to share that Care Synergy and the CAPABLE National Center have been selected as finalists for the Elevance Health Foundation Patient Safety Prize!

The Patient Safety Prize recognizes proven solutions that can help improve patient safety for communities across the United States. Care Synergy and the CAPABLE National Center were selected as finalists in the area of “Promoting Fall-Free Futures,” recognizing the potential of CAPABLE to help older adults remain safe, independent and engaged in their homes and communities.

Launched by the Elevance Health Foundation in December 2025, the Patient Safety Prize will award up to $5 million to innovative solutions addressing health literacy, medication errors and fall prevention.

As a finalist, Care Synergy and the CAPABLE National Center will have the opportunity to refine our project, incorporate feedback from the evaluation panel, and present our work to the Selection Committee.

Up to three winners will each receive up to $1.5 million, with up to two Honorable Mentions receiving up to $250,000. Winners and Honorable Mentions will be announced in fall 2026.

We’re proud to be recognized alongside four other finalists working to advance patient safety across the country, and we’re grateful for the opportunity to showcase how CAPABLE can help create safer, more independent futures for older adults.

Today, employees from Care Synergy, The Denver Hospice, Colorado Visiting Nurse Association, and Colorado PACE came together for a shared purpose: giving back to our community through blood donation.

Every blood donation has the potential to save lives, and we're proud of the team members who took time out of their day to help ensure lifesaving blood products are available for those who need them most.

The CAPABLE Implementation & Fidelity Survey is an annual survey of all CAPABLE implementation sites. The survey is a self-report on organizational characteristics and implementation experience. It provides a structured way to consistently:

• Capture program data (e.g., number of participants completed to date, cost per participant);
• Identify implementation barriers to and facilitators of CAPABLE;
• Document adaptations, staffing turnover, plans for continuity, funding sources;
• Confirm that the organizations are maintaining fidelity to the CAPABLE evidence-based protocol and terms of the license, through attestation by the program administrator.

For more than 135 years, Colorado Visiting Nurse Association has been helping Coloradans live healthier, more independent lives.

Our 2025 Annual Report highlights a year of transition, resilience, and continued commitment to community-based care. Through programs like CAPABLE and Medical Foot Care, CVNA continues to empower older adults to remain safe, healthy, and connected in their own homes and communities.

In 2025, CVNA:
✅ Served 131 individuals through the CAPABLE program
✅ Completed 3,797 Medical Foot Care patient visits
✅ Provided foot clinic services to 11,384 individuals

As we reflect on our history and look toward the future, we are grateful to our staff, volunteers, partners, supporters, and community members who make this work possible.
