The CAPABLE Implementation & Fidelity Survey is an annual survey of all CAPABLE implementation sites. The survey is a self-report on organizational characteristics and implementation experience. It provides a structured way to consistently:

• Capture program data (e.g., number of participants completed to date, cost per participant);
• Identify implementation barriers to and facilitators of CAPABLE;
• Document adaptations, staffing turnover, plans for continuity, funding sources;
• Confirm that the organizations are maintaining fidelity to the CAPABLE evidence-based protocol and terms of the license, through attestation by the program administrator.

For more than 135 years, Colorado Visiting Nurse Association has been helping Coloradans live healthier, more independent lives.

Our 2025 Annual Report highlights a year of transition, resilience, and continued commitment to community-based care. Through programs like CAPABLE and Medical Foot Care, CVNA continues to empower older adults to remain safe, healthy, and connected in their own homes and communities.

In 2025, CVNA:
✅ Served 131 individuals through the CAPABLE program
✅ Completed 3,797 Medical Foot Care patient visits
✅ Provided foot clinic services to 11,384 individuals

As we reflect on our history and look toward the future, we are grateful to our staff, volunteers, partners, supporters, and community members who make this work possible.

Programs like CAPABLE remind us that health care extends far beyond medical appointments.

In a recent story from UCHealth Today (https://www.uchealth.org/today/care-coordination-helps-patients-regain-stability/ ), a Colorado woman facing chronic pain, housing instability, and major life challenges was able to regain stability through coordinated support services — including home modifications and practical assistance provided through the CAPABLE program.

CAPABLE helps older adults remain safe and independent at home by addressing everyday barriers that can impact health and quality of life. Simple changes like accessible shelving, safer bathing spaces, and adaptive equipment can make an enormous difference.

Stories like this highlight the importance of whole-person care and the impact community-based programs can have on helping people maintain dignity, independence, and stability.

We’re excited to share that CAPABLE was recently featured in a new article from Money Magazine discussing how older adults can safely and comfortably age in place.

The article highlights the importance of home modifications and supportive services that help seniors maintain independence and quality of life as they age. CAPABLE — a program developed by the Johns Hopkins School of Nursing — was recognized for its innovative approach that brings together nurses, occupational therapists, and handyworkers to help older adults remain safely in their homes.

A new Medicare opportunity could help more older adults stay safe and independent at home—and Care Synergy and the CAPABLE National Center encourage healthcare organizations to apply.

The Centers for Medicare & Medicaid Services has launched the LEAD Model (Long-Term Enhanced ACO Design). Embedded within this initiative is RISE (Resilience and Independence in a Safe Environment), a falls-prevention program modeled after the evidence-based CAPABLE approach.

RISE is designed to help older adults:
• Reduce falls
• Maintain independence at home
• Perform daily activities like bathing, cooking, and dressing more easily

Healthcare organizations forming or joining an Accountable Care Organization (ACO) are encouraged to apply. By participating, organizations can help bring proven falls-prevention strategies into mainstream Medicare and improve quality of life for older adults.
