1. We are accepting NEW patients 14 years and under.
2. We have opened our patient portal for online scheduling, sick and well visits.
3. Check in text link will be sent out 24 hours prior to appointment, this must be completed prior to arrival.
4. We also ask that you arrive 15 minutes prior to appointment to get checked in.
Also, If you have not been seen in over a year, please give us a call. We require yearly well visits to remain a patient.
