A patient called me convinced her Botox had stopped working. I told her to go to the ER immediately.

That wasn't Botox. That was Bell's palsy.

Bell's palsy is sudden, one-sided facial paralysis. You wake up and boom, one side of your face stops working. Brow won't raise. Eye won't close. Smile drops.

The first thing you have to rule out is a stroke. Always.

Once we confirm Bell's palsy, the treatment window is everything. You have 24-72 hours to start steroids and antivirals. Move fast, and 85% of patients make a full recovery.

Wait too long, and the story gets more complicated.

Not everyone is a good candidate for filler in the tear trough area, and that's okay. PRP is a powerful alternative worth knowing about.

As a collagen booster, PRP can improve skin texture, reduce fine lines, address some under-eye darkness, and even help with acne scarring. It can be injected with a microdroplet technique, used during microneedling, or applied topically after laser treatments like Fraxel.

The key? Patient selection and realistic expectations. If you have large fat pads or loose skin, rushing any treatment in the tear trough area can lead to complications. Sometimes the best approach is alternating between filler and PRP over time, not trying to fill everything at once.
