Vital Care Services offers caring, personalized and professional in-home care for medically fragile children who have Medicaid (through the GAPP program), or private pay. We serve eligible children under 21 years of age who are medically fragile and in need of medically necessary skilled nursing care and/or medically necessary personal care support. Overview
The Georgia Pediatric Program (GAPP) of
the Georgia Department of Community Health (DCH) serves children who are medically fragile and in need of skilled nursing care. The program provides services either in-home or in a medical day care as an alternative to full-time skilled nursing facility care or institutional setting such as a hospital. Medically Fragile Day Care
The day-care portion of the GAPP Program operates under a Home- and Community-based Waiver approved by the Center for Medicare & Medicaid Services (CMS). Skilled and non-skilled care are provided five days per week in a medically licensed day care facility. Medically Fragile Day Care includes therapies and skills building to improve the child’s independence. Georgia Pediatric In-home Nursing Program
The Georgia Pediatric In-home Nursing Program serves eligible members, under 21 years of age, based on medical necessity for skilled nursing. Children must be medically fragile, require skilled nursing care, and currently be receiving physician-ordered services to be considered eligible for this program. GAPP Members Served
Members served by GAPP must meet the same level of care as necessary for admission to a hospital or skilled nursing facility and be eligible for Medicaid.
