If You Build It, Will They Come? A Community-Clinical Linkage Model for Arthritis Care

During this Chronic Disease Connect webinar, subject matter experts will introduce the Public Health Framework for Collaborative Osteoarthritis Management and Wellbeing and share how one Community Care Hub put the model into practice.

Participants will examine findings from the pilot evaluation, including what the data revealed and the questions that remain unanswered. The session will conclude with a discussion of how Learning Health Networks can support scaling the model beyond pilot sites and the functions and conditions needed for successful expansion.

The GEAR Group on Reimagining Prevention has been moved to September. Don’t miss this opportunity to participate!

Generate, Educate, Activate, Respond (GEAR) Groups are peer-to-peer, case-based, action learning opportunities for state and territorial chronic disease practitioners. Participants can expand knowledge of pressing topics, develop leadership skills, and network with other public health professionals at state health departments.

This GEAR Group will explore new models and innovative strategies to address chronic disease prevention for the 21st century. The role of new technologies such as telehealth and GLP-1 medications, as well as the potential of cross-sector partnerships and novel funding strategies, will be key areas of focus.

The group will meet virtually on four consecutive Wednesdays (September 2, 9, 16 and 23).
