“Deep plane facelift” has become an increasingly common term, but not every procedure described that way involves the same degree of release, repositioning, fixation, or neck work. The details of the operation matter.

The same is true when comparing surgery with nonsurgical treatments. Threads may create temporary repositioning, and Sculptra can be useful for restoring volume, but neither reproduces the structural changes achieved through a properly performed facelift. Volume replacement, skin tightening, and surgical lifting are three very different things.

Jawline and neck definition can also depend on structures beneath the skin, including the platysma and deeper tissues. Simply tightening the skin does not necessarily address the underlying cause of facial aging.

Advanced facial surgery takes years of specialized training, mentorship, anatomical study, and repetition. Technique matters, but so does the surgeon performing it.

The goal should be to understand what is actually being done, why it is being recommended, and whether it appropriately addresses your anatomy and goals.

Why can an 85° lift create significant tension without making the face look overly tight?

Because the direction of that tension matters.

In facial rejuvenation, an overly pulled appearance is often associated with excessive horizontal tension. When tissues are pulled backward toward the ears, the face can begin to look stretched rather than naturally elevated.

Dr. Stong’s approach is based on an almost completely vertical vector of repositioning, which is what the 85° angle represents.

The goal is not to use less tension. It is to place that tension in a direction that more closely restores the tissues upward rather than backward.

With an extended deep plane facelift, the deeper facial tissues are first released from the retaining ligaments that are restricting movement. Once those structures are adequately mobilized, they can be repositioned vertically without relying on the skin itself to create the lift.

That distinction is important.

Horizontal pull = stretched appearance
Vertical repositioning = elevated facial structure

The face can still be securely lifted and supported, but the tension is directed upward rather than across the face.

That is one of the key principles behind creating meaningful rejuvenation while preserving natural facial expression and avoiding the classic “pulled” facelift look.

Eyelid rejuvenation is not simply about removing “extra skin” or eliminating under-eye bags. The upper and lower eyelids age differently, and each requires a different approach.

Upper blepharoplasty addresses excess skin and hooding to create a more open, refreshed appearance while preserving the natural shape of the eye. The incision is carefully placed within the natural eyelid crease, where it can heal discreetly.

For the lower eyelid, the issue is often not just “too much fat,” but where that fat is sitting. Prominent lower-eyelid fat can create under-eye bags, while a hollow beneath it can make the lid-cheek junction look more pronounced.

With lower blepharoplasty and fat transposition, Dr. Stong repositions the patient’s own fat into areas of volume deficiency rather than simply removing it. This helps soften under-eye fullness while creating a smoother, more natural transition from the lower eyelid into the cheek.

The goal is a refreshed, balanced result that still looks natural and maintains the character of the eyes.

Brow position is also an important part of the evaluation, since a lower brow can contribute to upper-eyelid heaviness and may need to be addressed for the most balanced result.

What actually creates a sharper, more defined neck angle during facelift surgery?

For Dr. Stong, it is not simply about pulling the skin tighter. One of the most important components is how the deeper structures of the neck are released, reshaped, and repositioned.

In this intraoperative teaching moment, Dr. Stong explains how his neck technique incorporates both central and lateral platysmaplasty to improve the cervicomental angle, the angle created where the underside of the chin transitions into the neck.

Centrally, the platysma muscles are addressed with a platysmaplasty and extended myotomies. These strategic muscle releases help free restrictive tension within the neck so the tissues can be repositioned more effectively.

Laterally, an additional myotomy and platysmaplasty allow Dr. Stong to create greater tension and directional control across the neck. This provides more torque at the cervicomental angle, helping create a cleaner transition beneath the chin and stronger definition through the jawline and neck.

This deeper repositioning can also help elevate the submandibular glands, which may contribute to fullness beneath the jawline.

One patient. Two stages of facial rejuvenation. A transformation that continues to evolve naturally over time.

At age 62, this patient was photographed 4 months after her KalosLift™ and upper blepharoplasty. At this stage, you can already see a more defined jawline and neck, improved facial contour, and a refreshed appearance around the upper eyes, while still looking completely like herself.

What helps make KalosLift™ results long-lasting?

A durable facelift result is not created by simply tightening the skin. The deeper facial tissues have to be properly released, repositioned, and securely supported.

During the KalosLift™, Dr. Stong performs a complete release of the deeper muscle and SMAS layer so the tissues can be elevated without relying on tension at the skin surface. Once repositioned, the SMAS flap is secured using periosteal fixation—anchoring the deeper tissues to the strong, stable layer covering the bone.

Fixation is performed at key structural points, including the mastoid bone behind the ear and the zygomatic arch and cheekbone region.

Why does this matter?

The periosteum provides a firm, stable foundation for fixation. By anchoring the repositioned muscle and SMAS to these deeper structures, the lift is supported at the anatomical level where the correction is actually being made rather than asking the skin to hold everything in place.

At 51 and about 7 weeks after her KalosLift™ and upper blepharoplasty, this patient shared something that perfectly captures what facial rejuvenation can mean on a personal level.
