I'm taking a break and spending time with family. So I'm putting a few reposts out to keep content consistent. I filmed this last year in Indianapolis. It was a great opportunity to look at real anatomy and get insight into the pathophysiology of a condition we see often in EMS and emergency medicine- Atrial Fibrillation.

Using a bougie in every intubation is best practice. Not necessarily because it will help with every intubation, but because making it part of your practice in every patient creates a process that you've mastered when that bougie is actually needed to facilitate the intubation. Let's review data on the bougie.

THE BLADE IS NOT THE STORY HERE! It's still an awesome blade, but checkout Intublade's new software platform! Intublade is leading the way in integrating QA/QI into an entire intubation platform. And there is even more exciting tools to assist in QI coming to the platform. As someone who is passionate about airway management, resuscitation, and education, I'm really happy to see a tool like this that can assist in improving our performance in intubation.

“Your scientists were so preoccupied with whether or not they could, they didn’t stop to think if they should." That was my first reaction to this device and the ability to provide flow through it. I first saw this device 3 years ago and I've been thinking about it constantly. The more I think about it, the less I'm convinced I have a good reason to be averse to it. Why not have any device introduced to the airway during an intubation have the capability to provide flow? This is something I REALLY want to monitor feedback on, and so do the manufacturers!

And my research project is...!
Mechanic CPR VS manual CPR for intra-arrest transport prior to ROSC. This is adult non-traumatic arrest that had a 911 response. I would love to bring the data, but unfortunately, it's going to be a while. Not only because cardiac arrest is so complicated, but because you don't just want to blast out your data and interfere with opportunities to put it on display and get it published. But I think this is an important question to ask. The study question itself may have an intuitive answer, but those numbers may give us a glimpse as to what happens to the quality of a resuscitation once you get moving no matter what type of CPR is being performed. Thanks SO MUCH to ESO for allowing me to get this project off the ground!

The 5 year recruitment period has ended and this trial is going to get going. This is standard of care, Epi 1mg q3-5 VS capping Epi at 2mg.

A pivotal clinical trial comparing the usual dose of epinephrine to a lower dose during the treatment of adults experiencing an out-of-hospital cardiac arrest.

Something I see quite frequently, particularly with discussions on resuscitation practices prior to intubation and all the steps to be taken to ensure success, is pushback against these processes. It usually looks something like "Just intubate, lol". We humans don't like to be wrong. We don't like to change. Doing things the way we've always done them is the "Comfort Zone of Wrong". Adam Grant has a whole chapter in his book "Think Again" dedicated to falling in love with being wrong". A principle that made a HUGE impact on my life. And hopefully this had an impact on my patients' lives as well.

I wish we could live-stream PCRF. Being able to be a fly in the wall for tough questions with maybe no answers, watch research dreams being born and then withering on the vine (This time), making a pitch before prolific researchers and statisticians, organizing and whiteboarding, making progress only to discover missing data sets...all this only to produce an abstract. Getting a glimpse into it would really enlighten people who think research is just someone on a laptop comparing two data sets and being done by lunch.
