"Just get good" is a flex that fails to understand the barriers that stand in the way of competency. It isn't possible or reasonable for a provider who did not intubate in Paramedic school, intubates infrequently in the field, and has no access to an OR on their own to become competent through some sort of bootstrapped initiative.

Does this paper demonstrate that Dual Seqential Defibrillation isn't effective in terminating refractory ventricular fibrillation? Only if the outcome was based on the intervention being applied 15-20 minutes deep and 6 shocks into the code on a 100 MINUTE RESUSCITATION! We have one RCT that has already demonstrated a benefit at under 4 shocks IN Canada. We have two other RCTs in the works. Those two RCTs will be very informative.

Normal Saline bad, right? Blood good? We've kind of painted ourselves into a corner with crystalloids, according to Dr. Mark Piehl. Crystalloids may be just fine as a resuscitation fluid for many patients, specifically if they don't need large volumes, but just enough to dial in a blood pressure we can work with. This is especially true for pediatric patients vulnerable to even brief moments of hemodynamic instability in pathologies like head injuries.

This was a small observational study. So maybe it's not representative of the EMS population as a whole. But what this study highlights is the importance of proper laundering of uniforms, and the importance of hand washing/sanitizing and appropriate PPE. If our nares are populated with staph, VRE, and C Dif, we have to consider ourselves a greater contamination risk to our patients than they are to us.

A clip I shared about New Orleans EMS administering packed red blood cells with no warming caused quite a stir. I also posted some data from New Orleans that they provided supporting the practice. But that's one voice making a claim, and the same voice offering support. Kevin is a leading voice in EMS driving evidence-based practice, and I knew he would have some thoughts on this. This runs a little long, but I think it's worth hearing put completely.

Really fast Afib rates are often driven by accessory tracts, yet we often say that if a rhythm is too fast to differentiate Afib to give Adenosine to slow it down. So we may very well be putting patients in harms way with this practice. In reality, if you can't rule out Afib from a fast SVT, then Adenosine is probably not the way to go.
