When an IVF cycle fails, it can feel devastating. I sit with patients every day for what we sometimes call the “WTF visit” as we go through every data point to understand what happened and what we can learn moving forward.

One of the most important things I want people to understand is this:
IVF failure is not a personal failure. It is a biologic outcome.

A failed cycle does not mean your body “failed” or that there is no hope. It means we now have more information about egg quality, s***m, embryo development, implantation, and how your body responded to treatment.

My goal in this week’s episode of the As A Woman Podcast is to help you understand the science, take the blame off yourself, and give you a roadmap for what to do next.

The best time to conceive is not just one day in your cycle.

Your fertile window is usually the 5 days leading up to ovulation and the day of ovulation itself. This timing matters because s***m can survive in the reproductive tract for up to 5 days, while the egg only survives about 12 to 24 hours after release.

That means the goal is not to guess the exact moment of ovulation, but to consistently cover the window when conception is actually possible.

A helpful approach is having in*******se every 1 to 2 days during your fertile window. This supports s***m quality and ensures s***m are present when ovulation happens.

Cycle awareness takes the pressure off timing everything perfectly and replaces it with strategy and consistency.

The HSG (hysterosalpingogram) is a test used to evaluate the inside of your uterus and fallopian tubes. During the procedure, a thin catheter is inserted through the cervix, and dye is gently pushed into the uterus and through the fallopian tubes while X-rays are taken.

We do this procedure to see if the uterus is clear and if the fallopian tubes are open. Think of it as a road map. We want to know if there are any blockages, abnormalities, or areas the dye can’t pass through.

One of the biggest things I am seeing in clinic impacting fertility right now is cannabis use in men.

A lot of people assume fertility is mostly about the female partner, but s***m health matters just as much. Regular cannabis use has been associated with lower s***m counts, reduced motility, abnormal s***m shape, and increased s***m DNA fragmentation. We are also seeing higher miscarriage rates in female partners and poorer IVF outcomes, including fewer eggs retrieved and lower fertilization success.

What surprises many couples is that even when semen analysis looks “normal,” s***m quality at the DNA level can still be affected.

If you are trying to conceive, this is a conversation worth having. Fertility is a couple’s health issue, not just a women’s health issue. Small lifestyle changes on both sides can make a meaningful difference in outcomes.
