Frozen shoulder management requires both medical and mechanical intervention.

Physicians address systemic contributors and intra articular inflammation. Physical therapists address capsular mobility, arthrokinematics, neuromuscular control, and progressive loading.

Medication alone does not restore capsular length. Mobility alone does not address inflammatory modulation or metabolic contributors.

Cortisone works best when inflammation is the main driver of pain. In early frozen shoulder, the capsule is irritated and filled with inflammatory chemicals. That is why pain can improve quickly after an injection.

But as the condition progresses, the problem shifts from inflammation to fibrosis. Fibrosis means the capsule thickens and tightens due to excess collagen buildup.

In that stage, reducing inflammation alone will not restore motion. The capsule has to be gradually remodeled with guided loading and joint mobilization.

Pain relief is helpful. But motion restoration requires movement.
