EXPLAINER: What Is Molecular Hydrogen and Why Are Researchers Paying Attention?
If you follow longevity research, you've probably seen molecular hydrogen (H2) mentioned with increasing frequency. Here's a primer on why it's generating interest in the clinical community.
What it is: Molecular hydrogen is the simplest and smallest molecule in existence — two hydrogen atoms bonded together. When dissolved in water and consumed, it acts as a selective antioxidant.
Why 'selective' matters: Unlike traditional antioxidants (vitamin C, E, resveratrol) which neutralize all free radicals indiscriminately, H2 specifically targets hydroxyl radicals and peroxynitrite — the two most destructive reactive oxygen species in human biology. It leaves beneficial free radicals (needed for immune signaling and exercise adaptation) untouched.
Why size matters: At just 2 atomic mass units, H2 crosses every biological membrane in the body, including the blood-brain barrier and mitochondrial membranes. No other antioxidant molecule can make this claim.
The research: Over 2,000 peer-reviewed studies. 80+ registered clinical trials. Research spanning cardiovascular protection, neuroprotection, anti-inflammation, metabolic health, and exercise recovery. Published in journals including Nature Medicine, Free Radical Research, and the Journal of Clinical Biochemistry and Nutrition.
What to look for: Not all H2 products are equal. Concentration (measured in PPM) varies dramatically. The clinical studies showing results used 8-12 PPM. Many commercial products deliver 2-3 PPM. Third-party verification and IHSA certification are the most reliable indicators of quality.

THE STATIN PARADOX
In 2023, statins were prescribed to over 40 million Americans. They are the most prescribed drug class in the United States.
They work. LDL goes down. That's measurable and reproducible.
But here's the paradox:
→ Up to 50% of all heart attacks occur in patients with 'normal' LDL levels
→ 60% of long-term statin users report muscle symptoms significant enough to affect daily life
→ Statins reduce CoQ10 production by up to 40% — the energy molecule your heart muscle depends on
→ Standard lipid panels don't test for oxidized LDL — the form that actually causes arterial plaque
This doesn't mean statins are useless. For certain high-risk populations, the evidence for benefit is strong.
But it does mean that for millions of patients, we may be managing a metric while missing the mechanism.
The question isn't 'should we lower LDL?' The question is 'should we ALSO be addressing the oxidation that makes LDL dangerous?'
Emerging research on selective antioxidant therapy — particularly molecular hydrogen — suggests the answer is yes.

5 Things Your Standard Cholesterol Panel Doesn't Measure (But Should)
Your doctor orders a lipid panel. You get your results. Total cholesterol, LDL, HDL, triglycerides. Four numbers. A thumbs-up or a prescription.
Here's what that panel leaves out:

Oxidized LDL — the form of cholesterol that actually initiates plaque formation. Regular LDL is a transport molecule. Oxidized LDL is an inflammatory trigger. They're fundamentally different substances, and your standard panel doesn't distinguish between them.
LDL Particle Size — small, dense LDL particles are significantly more atherogenic than large, buoyant ones. Two patients with identical LDL numbers can have vastly different risk profiles based on particle size alone.
Lipoprotein(a) — a genetically determined cardiovascular risk factor that affects roughly 20% of the population. It's not modifiable by diet or statins, and it's almost never tested.
hs-CRP — a marker of systemic inflammation that independently predicts cardiovascular events, even in patients with 'normal' cholesterol.
RBC Magnesium — the mineral required for over 300 enzymatic processes including cholesterol metabolism. 75% of Americans are deficient. It's almost never included in a standard panel.

The four numbers on your lipid panel are a start. They're not the whole story.
Ask better questions. Request better tests.
