✨ WHY THE DAO MUSCLE MATTERS ✨

💉 One of the most important muscles we evaluate during a Full Face Botox treatment is the Depressor Anguli Oris (DAO).

📍 As we age, the balance between the muscles that lift the face and the muscles that pull the face downward begins to change.

⬆️ Above the mid-face line, we primarily have elevator muscles.
⬇️ Below it, we have depressor muscles.

Over time, these depressor muscles often become stronger and start pulling the tissues downward.

🔗 Remember: muscles connect to fascia, and fascia connects to the skin. Everything is connected.

This is one of the reasons we begin to see:

🔸 Drooping cheeks
🔸 Downturned corners of the mouth
🔸 Loss of facial support
🔸 A heavier appearance in the lower face

✨ When the DAO muscle is properly relaxed, the elevator muscles can function more freely, helping support and lift the face naturally.

⚠️ However, precision is critical.

The lower face contains several important muscles that must be carefully avoided, including the depressor labii inferioris and portions of the orbicularis oris. Injecting the wrong muscle can affect facial movement and symmetry.

👄 Lip Filler: Why Technique and Anatomy Matter

Today we want to discuss an important topic that creates a lot of confusion in aesthetic medicine: lip filler migration and the development of so-called “hyaluronic mustaches.” Many patients assume that migration suddenly occurs after one treatment or that the most recent injector caused the issue. In reality, the process is often much more complex.

🧬 Lip anatomy plays a critical role
The lips contain multiple anatomical structures surrounding the vermilion border (the natural border between the lip and surrounding skin). Depending on injection technique and filler placement, certain anatomical pathways can allow filler to gradually spread beyond the desired lip contour.

When filler repeatedly enters these pathways over multiple treatments, the process can be progressive:

▫️ Initial treatment → changes may be minimal and nearly invisible
▫️ Additional treatments using similar placement patterns → accumulation may increase
▫️ Over time → visible migration may become more apparent

Potential findings may include:
• Fullness above the natural lip border
• “Hyaluronic mustache” appearance
• Shadows or irregular contours
• Upper lip projection outside natural proportions
• Development of a “duck lip” appearance

📈 Migration may develop gradually over time
Patients commonly say: “My last treatment caused this.” However, in many cases, migration may represent gradual accumulation occurring over multiple sessions rather than a single isolated event.

💉 The filler itself is not always the cause

Denser fillers do not automatically create unnatural results.
Patients may receive different fillers over many years and still maintain natural anatomy when placement is carefully controlled.
🔬 Why movement recommendations matter

After treatment, we recommend limiting excessive lip movement for about 36 hours. Not because filler suddenly moves above the lip contour, but because the orbicularis oris muscle may influence how filler settles and affect the final shape.

Many results you see online are created by placing filler into the soft tissue using a cannula. This can give an immediate “snatched” look—but over time, due to gravity and skin laxity, those boluses can shift lower… and the definition doesn’t last the way you expect.
