If your child requires school forms to be filled out please drop them off at your earliest convenience or contact the office to see if they require an office visit. The typical turnaround time is 5-7 business days and we ask that you please pre-fill all patient/parent information beforehand.
A fee of $25 will be applied for the first signature page, and $5 thereafter for any additional forms.
