When is myasthenia gravis treatment working as well as it should?

For people living with MG, the answer is not always obvious. Symptoms can fluctuate, routines can quietly change, and patients may begin adjusting to limitations without realizing how much their condition is affecting daily life.

In a recent Everyday Health article, Ricardo Roda, MD, PhD, Director of the Johns Hopkins Myasthenia Gravis Center, discusses why changes in vision, speech, swallowing, fatigue, limb strength, breathing, or daily activity can be important signals to track.

For more than a century, ketogenic diet therapy has remained one of the most fascinating tools in epilepsy care.

Developed in the 1920s to mimic the metabolic effects of fasting, the ketogenic diet can be highly effective for some patients with drug-resistant epilepsy. Yet even after 105 years of clinical use, researchers are still working to understand exactly why it works.

In a recent Medscape Medical News feature, Eric Kossoff, MD, pediatric neurologist and director of the ketogenic diet program at Johns Hopkins, helped explain why. The diet appears to influence multiple overlapping systems, including brain energy metabolism, excitation and inhibition, and possibly even the gut microbiome.

That complexity is also why a simple “keto pill” has remained elusive. As Dr. Kossoff notes, the benefit likely comes from the interplay of many mechanisms rather than a single pathway.

The article is a reminder that some of medicine’s most durable therapies still hold unanswered questions, and that continued research may help make effective treatments more practical, personalized, and accessible for patients with epilepsy.

What if stroke treatment could be guided by a more precise view of the brain’s most vulnerable tissue?

Johns Hopkins researchers, led by Richard Leigh, M.D., are studying advanced imaging methods that may help clinicians better understand the “leaky core” of an ischemic stroke, an area where disruption of the blood-brain barrier may offer important clues about injury, treatment response, and recovery.

This work reflects a larger shift in stroke research: moving toward more precise ways to see what is happening in the brain, so treatment and recovery strategies can become more targeted over time.

For Johns Hopkins Neurology, it is another example of the stroke ecosystem at work: imaging, vascular neurology, acute care, recovery, prevention, and research moving together.
