House Call Physician of the Year: for the physician setting the bar on quality, innovation, and staying power in the field.

House Call Educator of the Year: for the educator shaping the next generation of house call clinicians.

House Call Clinician of the Year: for the NP, PA, or social worker whose work changes what patients experience at home.

Dan Gilden Creative Investigator Award: for the researcher whose body of work is moving the whole field forward.

The next era of acute hospital care at home is here – are you ready? As HaH programs prepare to launch or scale, some of the requirements set out in the CMS Acute Hospital Care at Home waiver may create misperceptions that can delay a program’s launch or limit its expansion.

In this webinar, Hospital at Home Users Group leader Linda DeCherrie, MD, (DispatchHealth) will be joined by Ashby Wolfe, MD, MPP, MPH, Regional Chief Medical Officer and the lead physician supporting the Acute Hospital Care at Home Initiative at CMS. The conversation will use clinical vignettes to address common questions related to transport and EMS coordination, clarify CMS requirements related to response time, discharge and in-home visits, home safety screenings, and caregiver considerations, among other essential topics. Importantly, the discussion will also explore the importance of clinical judgment and the operational flexibility that is built into the waiver’s requirements. This session is intended for HaH practitioners and leaders and is not open to the press.
