A patient said something to me recently that I hear all the time.

“I just don’t feel like myself anymore.”

She wasn’t talking about one symptom. She was talking about all of them.

Her cycles had gotten a little shorter. She was waking up around 4 a.m. a few nights a week. She felt more irritable than she used to and figured she was just stressed. The night sweats? She blamed the weather.

None of it seemed like much on its own. But when she said it all out loud, one after another, something clicked.

Perimenopause isn’t a switch that flips overnight. It shows up in pieces, a few small things at a time, until one day you notice you don’t quite feel like yourself anymore and you’re not sure why.

That’s what I’m actually listening for during a consultation. Not just what’s bothering you, but everything you’ve stopped mentioning because it didn’t seem worth bringing up. Any one of those symptoms on its own doesn’t tell me much. Together, they usually do.

One of the things I never want patients to feel is judged. If you enjoy a glass of wine, that’s your choice.

My job is simply to help you understand how your body works so you can make informed decisions, especially if you’re working on your hormones, your cycle, or trying to conceive.

Your liver has a lot of jobs. One of them is helping your body metabolize estrogen. When it’s also working to process alcohol, that process can become less efficient for a period of time.

Over time, that can contribute to things like:
• Heavier periods
• Worsening PMS
• Breast tenderness
• More irregular cycles

If you’re trying to conceive, alcohol can also influence egg quality and cycle regularity in ways that don’t always show up on routine lab work.

In Chinese medicine, we think about the Liver a little differently than Western medicine does. We see it as playing an important role in how smoothly things move through the body, including your cycle and your response to stress. That’s one of the reasons we talk about alcohol, sleep, nutrition, and stress together instead of as separate conversations. In Chinese medicine, we’re identifying patterns that lead us to the root cause of your symptoms.

One drink isn’t going to undo months of healthy habits. And we’re not looking for perfection. The hope is that you understand your body well enough to make choices that support your goals.

I hear that a lot during the first visit. You’ve been dealing with something for a year, sometimes longer, and when you finally come in for your intake, that’s one of the first things you tell me.

There’s no judgment in that. Most people try to manage on their own first. We hope things resolve on their own. We get busy. We’re not sure if what we’re experiencing is significant enough to make an appointment over.

It usually is. And the sooner we start looking at the whole picture, the more we can do to help.

These aren’t things you have to accept as normal.

Painful periods. Debilitating PMS. Exhaustion that doesn’t respond to rest. A year of trying with no answers. A diagnosis that basically means “we don’t know.“

These are all common patterns. And patterns have reasons.

Chinese medicine has been tracking patterns and documenting those reasons for a very long time. When something’s felt off and conventional care has run out of explanations, the patterns are usually where we start.

Pride month is winding down, but this isn't a once-a-year conversation for us.

We work with LGBTQ+ individuals and couples building their families in many different ways. If you're preparing for IUI, supporting a partner through IVF, using donor eggs or s***m, working with a surrogate, or simply trying to figure out what your next step looks like, you deserve care that meets you where you are.

One thing I believe deeply is that the intake shouldn't change based on who you love or how you're building your family. We take the time to understand your health, your goals, your story, and everything your body has been trying to tell us along the way.

One of the most common things I hear is: "My cycle is pretty normal."

Sometimes it is.

Sometimes it only looks that way because no one has ever asked about the other three weeks.

A patient came in after trying to conceive for over a year. Her period arrived every 28 days, lasted five days, and didn't seem particularly remarkable.

Then we talked about everything that happened before and after it.

Her energy disappeared after her period. She never noticed cervical mucous around ovulation. Her luteal phase was short. The week before her period felt like she hit a wall.

Her period looked normal. But the rest of her cycle told a different story.

I spend time asking questions that don't seem connected at first. Your energy, sleep, mood, and how your body shifts week to week all help tell the story of what's happening beneath the surface.

Your period is only one part of the conversation. We're listening to the whole month because the part that gets ignored is usually the part that we need to learn the most about.

Things patients almost don’t tell me.

A recent patient told me her cycle was “normal.”

When I asked about day one of her period specifically, she mentioned spotting for a few days before it actually starts. She didn’t think it was an important part of the story.

It is.

That’s one of the first things I want to know more about.

Pre-period spotting is something we pay close attention to. It can point to a few different patterns depending on what else is happening in the cycle, but it’s never nothing.

Which is one of the reasons why our intake takes the time it does.

The detail someone almost doesn’t mention is often the one worth exploring.

Juneteenth marks freedom that came late, and came unevenly, and is still being fought for in places most people don't think to look.

One of those places is the delivery room.

According to the CDC's most recent data, Black women in the United States die from pregnancy-related causes at more than three times the rate of white women. This isn't explained by genetics. Researchers consistently point to how Black patients' pain, symptoms, and instincts get dismissed by the people who are supposed to be caring for them. Here in Baltimore, we see what leads to it. Patients who've been gaslit, talked over, sent home before someone finally listened.

In 2009, I spent time collecting data for Baltimore Healthy Start, testing new mothers' stress markers in their own homes across the city. Organizations like theirs and MOMCares are still doing the work of changing these outcomes, one mother and one birth at a time. MOMCares is the only organization in Maryland providing no-cost doula services. Baltimore Healthy Start has been working to reduce infant and maternal mortality in our city since 1991.
