📚All students were sent home with one or both of the following handouts for School Based Telehealth services! If you intend to utilize School Based Telehealth this year, make sure that you are submitting the consent form to make the process easier and more efficient!

✏️To fill out and submit the School Based Telehealth consent form, scan the QR code on the paper below. This form can also be filled out by searching bambergfamily.com in your web browser and clicking on the Telehealth tab, where the form is linked as well.

School-Based Telehealth is an amazing resource for all Bamberg County Students! This program connects students to a licensed healthcare provider right from school, which reduces missed classroom time. Parents are also able to join the visit remotely, right from work! Using School Based Telehealth is super easy and convenient! It enables students to see one of our Telehealth Providers from their school nurses office, and parents can even join remotely from their job! Do not forget to sign a School Based Telehealth Consent so you and your family can use this amazing service that we offer our community!
