Aging Grace House Calls is growing! We’re looking for a compassionate Nurse Practitioner to join our geriatric house-call practice serving older adults in the Bangor area.

This is a great opportunity for an NP who values:

* 🏡 Providing care in patients’ homes
* 👵 Building meaningful, long-term relationships with older adults
* 🩺 Geriatric-focused primary care
* ❤️ More time with patients and less of the traditional office grind
* 🌿 A collaborative, supportive practice environment

Schedule: Flexible/part-time to start, with potential to grow.

Ideal candidate: NP with an interest in geriatrics, strong clinical judgment, and a passion for helping older adults remain healthy and independent at home.

🎉 Celebrating 2 Years of House Calls! 🎉

Tomorrow marks two years of providing geriatric primary care through home visits, and I’m incredibly grateful to the patients and families who have trusted us with their care. It started with an idea, one patient and a need in the community. It’s brought me immense joy to develop this practice.

As we look ahead to the new year, I’m excited to share that we’re growing our practice and hiring additional clinicians to help meet the complex medical needs of older adults—right where they’re most comfortable: at home.

• Chronic disease management
• Medication review
• Post-hospital follow-up
• Care coordination for complex health needs
