For eligibility, please call our front desk at 920-887-1766. Church Health Services, Inc., a 501c3 and a United Way agency, conducts primary, dental and mental health care clinics for low income children and adults who live in South Central Wisconsin. We also partner with local congregations to help them to develop congregational health ministry teams and manage Parish Nursing programs in participating congregations.
