Peptides: Promise, Hype, and What We Actually Know (continued)

There’s growing excitement around peptides and their potential to support healing, aging, and performance. In the future, we may be able to tailor precise peptide combinations to individual needs. For now, this space is somewhat of a “Wild West”—full of promising claims, but also limited regulation, incomplete research, and many unknowns.

In the May Newsletter, I discussed GHK-cu, BP157, and TB500.

In this newsletter, I will focus on Growth Hormone Releasing Hormone (GHRH, like sermorelin, ipamorelin, and tesamorelin), PT141, and KPV.

1. Growth Hormone–Releasing Hormone (GHRH)

(Examples: sermorelin, tesamorelin, ipamorelin)

These peptides work by signaling the body to produce more of its own growth hormone naturally. Sermorelin and tesamorelin are the most researched options and have been FDA-approved for certain medical uses. While initially used to help treat conditions involving muscle loss, they are increasingly utilized in wellness medicine to support healthy aging, body composition, recovery, and overall vitality.

What they may do:
• Increase muscle mass and bone density
• Reduce visceral fat
• Improve body composition
• May have favorable effects on cholesterol
• May improve sleep

Common usage patterns: Cycled dosing (e.g., 5 days on, 2 days off, AND 3 weeks on/1 week off) to reduce tolerance and ineffectiveness.

Risks & considerations:
• Can increase blood glucose levels and insulin resistance
• May potentially stimulate cancer growth, as growth hormone pathways can influence cell proliferation
• Long-term safety data remains limited outside of specific medical indications
• Joint and nerve pain
• Fluid retention (edema)

2. PT-141
PT-141 (Bremelanotide) is a synthetic peptide that acts as a non-selective melanocortin receptor agonist. It is FDA-approved under the brand name Vyleesi for treating Hypoactive Sexual Desire Disorder (HSDD) in premenopausal women and has also been heavily researched as an "on-demand" treatment for erectile dysfunction (ED) in men. It is difficult to get insurance to cover the medicine, but is easily compounded.

Key Benefits
• Boosts Sexual Desire: Stimulates neurons in the hypothalamus to increase libido and arousal.
• Treats Erectile Dysfunction (ED): Facilitates pe**le erections by acting centrally on the nervous system rather than relying strictly on peripheral blood flow.
• Research indicates it is a viable alternative for men who do not respond well to medications like sildenafil (Vi**ra).
• Rapid Onset: It typically takes effect within 45 to 60 minutes after subcutaneous injection, with effects lasting anywhere from 4 to 12 hours.

Known Risks and Side Effects (generally well tolerated)
• Nausea: The most frequently reported side effect.
• Cardiovascular Effects: Can cause a transient increase in blood pressure (approx. 2-6 mmHg) and a temporary decrease in heart rate.
• Hyperpigmentation: Darkening of the skin, gums, or existing moles can occur, particularly with frequent use (e.g., more than 8 doses per month).
• Facial flushing and headaches--usually mild.

3. KPV
KPV (Lys-Pro-Val) is a potent, naturally occurring tripeptide regarded for its targeted anti-inflammatory properties, capacity to stabilize mast cells, and ability to inhibit histamine release, all without broadly suppressing the immune system.

Key Benefits
• Histamine Reduction: KPV stabilizes mast cells and prevents mast cell degranulation, potentially offering marked relief from histamine intolerance, Mast Cell Activation Syndrome (MCAS), flushing, and allergic reactions.
• Systemic Inflammation: Research demonstrates its efficacy in calming inflammatory bowel disease (IBD) and respiratory inflammation.
• Gut Health: KPV promotes the repair of the intestinal epithelial barrier, aiding in the recovery of "leaky gut" and post-antibiotic dysbiosis.
• Skin and Tissue Healing: It accelerates wound healing, minimizes scar tissue, and soothes inflammatory skin conditions like eczema, psoriasis, and dermatitis

Primary Risks & Limitations
• Not FDA Approved: Because of the lack of human clinical trials. It is not approved by the FDA for any specific medical condition.
• Side Effects: When administered via subcutaneous injection, KPV can occasionally cause localized injection site reactions or mild, temporary flu-like symptoms.

Bottom Line
Peptides represent an exciting and evolving area of medicine with real potential—especially in healing, recovery, and aging-related therapies.

However:
• Many uses are not yet well studied in humans
• Products are often unregulated
• Long-term safety is still unclear
• Some mechanisms (like increased blood vessel growth) may carry theoretical risks, including tumor progression

In short: promising tools, but not without caution.

If you’re considering peptides, it’s important to have a thoughtful, individualized discussion about potential benefits, risks, and safer, evidence-based alternatives.

A recent study published in JAMA Network Open found that women with stage I–III breast cancer had significantly better 10-year survival outcomes when taking GLP-1 medications. Survival improved by 65% in women with obesity and by 56–91% in women with type 2 diabetes.

The reduction of hormones after menopause can cause some lack of sensitivity; adding hormones back helps to retain sensitivity to use this matured organ more fully!

3. Flibanserin (brand name Addyi) [by Sprout Pharmaceuticals] has been approved for hypoactive s*xual desire in premenopausal women—but not so fast…
It only helps 1 in 10 women and should be stopped after 8 weeks if the woman does not notice any benefits. It can seriously lower blood pressure, especially if you drink any alcohol. It should be taken at bedtime, and you should not drive or operate any heavy machinery within 6 hours of taking it. It can have serious drug interactions, especially with certain antibiotics and anti-yeast medications.

Our take: optimize testosterone first and maybe try the O-shot—a very minor procedure shown to improve s*xual sensation and lubrication without any serious side effects.
