PMOS, formerly PCOS, is often thought of as a reproductive condition.

But it's also closely connected to metabolic health.

According to the CDC, more than half of women with PMOS develop type 2 diabetes by age 40.

One reason is that insulin resistance is common in PMOS. Over time, insulin resistance can make it harder to regulate blood sugar and increase the risk of developing type 2 diabetes.

The encouraging news is that metabolic health can often be improved.

For many people, nutrition, physical activity, sleep, stress management, and individualized medical care all play a role.

Have you noticed a new acronym?

In May 2026, PCOS (Polycystic O***y Syndrome) was officially renamed PMOS (Polyendocrine Metabolic Ovarian Syndrome).

Why?

Because the old name didn't tell the whole story.

Many people with PCOS don't have ovarian cysts. More importantly, the name overlooked one of the condition's defining features: metabolic dysfunction, including insulin resistance.

The updated name reflects what researchers and clinicians have increasingly recognized: that this condition affects much more than reproductive health.

At ADS, we believe names matter because they shape conversations.

And better conversations lead to better understanding, earlier recognition, and more opportunities to improve metabolic health.

Type 2 diabetes rarely develops overnight.

Insulin resistance often begins years before blood sugar reaches the diabetic range.

During that time, the body is adapting.

By the time diabetes is diagnosed, metabolic dysfunction may have been developing for a decade or longer.

That's why prevention matters.

Understanding insulin resistance gives us an opportunity to identify risk earlier while simultaneously supporting better health before chronic disease progresses.

Many people think of insulin as a hormone that only matters if you have diabetes.

In reality, insulin helps regulate how your body stores and uses energy, and when cells become resistant to insulin, the effects can extend far beyond blood sugar.

Growing evidence links insulin resistance with conditions such as type 2 diabetes, metabolic syndrome, nonalcoholic fatty liver disease (MASLD), cardiovascular disease, polycystic o***y syndrome (PCOS), which is now called polyendocrine metabolic ovarian syndrome (PMOS) to more accurately reflect the condition's complex hormonal, metabolic, and systemic nature, and more.

Researchers are also studying its role in cognitive decline, certain cancers, and other chronic conditions.

That's why improving metabolic health isn't just about lowering glucose, but addressing one of the underlying processes that affects health throughout the body.

Understanding insulin resistance is one of the first steps toward changing the trajectory.

Knowledge is empowerment and prevention is key. Both of these are powerful steps towards lifelong health.

Insulin resistance, estimated to affect nearly 40% of Americans ages 18–44, often develops long before type 2 diabetes is diagnosed.

Research suggests it can begin 10–15 years before blood sugar reaches the diabetic range. During that time, many people may feel well (or experience subtle signs such as increased hunger, weight gain around the midsection, fatigue, or rising triglycerides) without realizing that metabolic health is changing.

That's why waiting until blood sugar becomes abnormal can mean missing years of opportunity for prevention.

The good news is that insulin resistance is often responsive to lifestyle changes.

For many people, improving food quality, finding an appropriate level of carbohydrate intake, increasing physical activity, prioritizing sleep, and managing stress can all support better metabolic health over time.

A1C estimates average blood sugar over the past 2–3 months by measuring how much glucose has attached to red blood cells. But A1C can also be influenced by factors such as red blood cell lifespan, iron status, and certain medical conditions.

That means two people with the same A1C may have very different metabolic health.

A normal A1C doesn't always rule out insulin resistance.

And an elevated A1C doesn't always tell the full story either.

That's why understanding your metabolic health may involve looking beyond a single number. Depending on your individual situation, your healthcare provider may consider additional markers such as fasting glucose, fasting insulin, triglycerides, HDL cholesterol, or other assessments of insulin resistance.

The goal isn't to memorize lab values.

It's to understand what they're telling you—and what they aren't.

A normal A1C doesn't always mean everything is normal.

A1C is a valuable tool for assessing long-term blood sugar, but it's only one piece of the metabolic health puzzle. Factors such as red blood cell lifespan, iron status, inflammation, and certain medical conditions can influence A1C, meaning it may not always reflect what's happening beneath the surface.

If you're experiencing persistent fatigue, difficulty managing your weight, brain fog, or other signs of metabolic dysfunction despite a normal A1C, it's worth having a conversation with your healthcare provider about whether additional evaluation is appropriate.

Depending on your individual health history, that may include looking at markers such as fasting glucose, fasting insulin, triglycerides, HDL cholesterol, post-meal glucose, or other assessments of insulin resistance.

A1C is an important marker but it's only part of the picture.

For many people, A1C is the first number they hear when talking about diabetes or blood sugar, and while it's a valuable tool, it doesn't tell us everything about metabolic health.

A1C estimates your average blood sugar over the previous 2–3 months, but it doesn't directly measure insulin resistance, which is the metabolic dysfunction that often develops years before type 2 diabetes is diagnosed.

That's why two people can have the same A1C and very different metabolic health.

Looking at additional information—such as fasting glucose, fasting insulin, triglycerides, HDL cholesterol, and how your body responds to meals—can provide a more complete picture. Your healthcare provider can help determine which tests are appropriate based on your individual health history.
