I’m incredibly honored to have an article published in HomeCare Magazine.

A chart can be complete and still not tell a consistent story.

That’s something we see often in home health. The information may be there, but when you look across the OASIS, visit documentation, Plan of Care, diagnoses, interventions and goals, the pieces don’t always support one another.

And sometimes, it isn’t that the clinician didn’t provide the care. It’s that the documentation didn’t clearly reflect it.

In reality:
✔ Patient educated
✔ Medications reviewed
✔ Care coordinated
✔ Plan updated
✔ Problems addressed
✔ Progress assessed

In the chart:
“Patient tolerated visit well.”

😅😅😅

We know the care happened.

But if the chart doesn’t show it… compliance, coding, OASIS, reimbursement, and audit reviewers are all sitting there like:

“Okay, but where?” 👀
