One of the things I'm most excited about this year's Harvard Medical School course is our expanded focus on women's hormone therapy and s*xual health. Alongside an incredible faculty — including Dr. Kelly Casperson, Dr. Corinne Menn, and Dr. Marianne Brandon — we'll tackle the evidence behind menopause, HRT, testosterone therapy in women, s*xual dysfunction, breast cancer survivorship, and so much more.

I am SO excited to welcome Dr. Talia Sobel to our practice — and we are incredibly lucky to have her.

Dr. Sobel is a board-certified internal medicine physician with advanced fellowship training in women’s health from Johns Hopkins. She previously served as an Assistant Professor of Medicine in the Division of Women’s Health at Mayo Clinic Arizona.

Why does this matter? Because women in midlife deserve more than fragmented care. Menopause does not happen in a vacuum. Hormones, heart health, bone health, metabolism, sleep, mood, s*xual health, cancer survivorship — it is all connected.

Dr. Sobel understands the whole picture. She is brilliant, thoughtful, compassionate, and deeply committed to listening to women and helping them feel better.

Those who got vaginal estrogen prescriptions had major decreases in

Hospital admission
Sepsis and
Death

60-80% lower odds of death ☠️🪦⚰️

Giving vaginal estrogen is in the guidelines and yet only 5% of patients were given prescriptions.

We have a lot of work to do and need to get the word out

Vaginal Hormones:
- prevent urinary tract infections, sepsis and death
- fix urinary frequency, urgency, and leakage
- they help lubrication, pain with s*x and satisfaction
- they have NEVER been shown to cause cancer of any kind or blood clots

As of 2025 there are now guidelines in place to teach out how and why

A newly published study of nearly 1.9 million women found that women with recurrent urinary tract infections who were prescribed vaginal estrogen had:

✅ Lower rates of sepsis
✅ Fewer hospitalizations
✅ Lower all-cause mortality

That's an incredible finding.

Now, let's be clear about what this study does—and doesn't—show.

This was an observational study, so it does not prove that vaginal estrogen caused these better outcomes. The women who received vaginal estrogen may also have received better overall medical care. The authors discuss this important limitation.

But here's what we already know from years of high-quality research:

✔️ Vaginal estrogen is one of the most effective treatments for Genitourinary Syndrome of Menopause (GSM).

✔️ It helps restore the health of the vaginal and urinary tissues.

✔️ It reduces recurrent urinary tract infections in many postmenopausal women.

✔️ It is recommended by major medical guidelines.

Unfortunately, far too many women are never offered this treatment because of outdated fears and misconceptions about estrogen.

If you've been struggling with repeated UTIs, vaginal dryness, painful in*******se, urinary urgency, or bladder symptoms after menopause, don't assume it's "just aging."

These symptoms are common—but they are treatable.

💜 Women deserve evidence-based care.

💜 Women deserve clinicians who understand menopause.

💜 Women deserve to know all of their options.

Saw Suffs the musical last night and ugly cried the whole show.

It’s about the history of Alice Paul and the team of suffragists who spent years and years fighting for the right for women to vote.

These fights are long. Hard. And nobody ever agrees on the details of how to get to the finish line.

And once you get to the finish line?

There’s so much more work to do.

More fights to have.

There’s a lot of drama between the old guard who wanted control and things done a certain way and the new guard who wanted to take a more aggressive and radical approach.

This is still happening today. In many many fights we are still fighting.

It was extra wild to run into Dr. Cheryl Iglesia - a famous urogynecologist who was on the original paper in 2014 describing genitourinary syndrome of menopause.

It took 11 years for us to get guidelines written and decades to get warning labels removed from estrogen products.

Dr Iglesia has made her career in the academic ladder climbing and paving the way for many of us. She has worked to train hundreds of urogynnecologists.

She and I have had our moments over the years. We take very different approaches to the problems ahead.

But I am here because of strong women like her. Whether we always agree or not. I am grateful.

Last night we both felt like Great American B*tches.
