From rural towns to urban neighborhoods, Community Health Centers are there, serving patients of all demographics.

This includes veterans, people without housing, older adults, and expectant parents.

In 2025 alone, health centers provided prenatal care to more than 618,000 patients. Nearly three-fourths were seen in the first trimester, a critical window for healthy outcomes.

Early and consistent prenatal care at health centers is proven to improve birth outcomes like higher birth weights and fewer preterm births.

That’s what it means to meet people where they are — with care they can trust, no matter their circumstances.

In 2025 alone:

✅ 3.7M patients received diabetes care — 73.6% reached A1c control
✅ 6.4M were treated for hypertension — 68.9% achieved blood pressure control

Building innovative care where it matters most means addressing the root causes of chronic disease, not just treating symptoms.

As we celebrate National Health Center Week's focus on health conditions, we're highlighting how Community Health Centers are leading the way through whole-person, evidence-based care.

Through our growing collaboration with the American College of Lifestyle Medicine (ACLM), NACHC is helping expand whole-person, evidence-based care that addresses the root causes of chronic disease—from workforce training and Food for Health initiatives to practical implementation tools that improve patient outcomes.

Together, we're advancing healthier communities by transforming how chronic conditions are prevented and managed.
